Musculoskeletal Anatomy, Physiology, and Assessment
Before discussing the concept of mobility, it is important to understand the anatomy of the musculoskeletal system, common musculoskeletal conditions, and the components of a musculoskeletal system assessment. Mobility and Immobility
Mobility is the ability of a client to change and control their body position. Physical mobility requires sufficient muscle strength and energy, along with adequate skeletal stability, joint function, and neuromuscular synchronization. Anything that disrupts this integrated process can lead to impaired mobility or immobility.[1] Mobility exists on a continuum, ranging from no impairment (i.e., the client can make major and frequent changes in position without assistance) to being completely immobile (i.e., the client is unable to make even slight changes in body or extremity position without assistance). See Figure 13.1[2] for an image of a client with impaired physical mobility requiring assistance with a wheelchair.
Functional mobility is the ability of a person to move around in their environment, including walking, standing up from a chair, sitting down from standing, and moving around in bed. The three main areas of functional mobility are the following:
- Bed Mobility: The ability of a client to move around in bed, including moving from lying to sitting and sitting to lying.
- Transferring: The action of a client moving from one surface to another. This includes moving from a bed into a chair or moving from one chair to another.
- Ambulation: The ability to walk. This includes walking independently but with assistance from another person or an assistive device, such as a cane, walker, or crutches.
Immobility can be caused by several physical and psychological factors, including acute and chronic diseases, traumatic injuries, and chronic pain. Several neurological and musculoskeletal disorders can adversely affect mobility, including osteoarthritis, rheumatoid arthritis, muscular dystrophy, cerebral palsy, multiple sclerosis, and Parkinson’s disease. Traumatic injuries, such as skeletal fractures, head injuries, or spinal injuries, also impair mobility. Diseases that cause fatigue, such as heart failure, chronic obstructive pulmonary disease, and depression, or conditions that cause pain also affect the client’s desire or ability to move.
Effects of Immobility
Clients who spend an extended period of time in bed as they recover from surgery, injury, or illness can develop a variety of complications due to loss of muscle strength (estimated at a rate of 20% per week of immobility). Regardless of the cause, immobility can cause degradation of cardiovascular, respiratory, gastrointestinal, and musculoskeletal functioning. Promoting mobility can prevent these complications from occurring. Findings from a literature review demonstrated several benefits of mobilization, including less delirium, pain, urinary discomfort, urinary tract infection, fatigue, deep vein thrombosis (DVT), and pneumonia, as well as an improved ability to void. Mobilization also decreased depression, anxiety, and symptom distress, while enhancing comfort, satisfaction, quality of life, and independence.[3] See Table 13.2a for a summary of the effects of immobility on these body systems.[4],[5],[6] Decreased mobility is also a major risk factor for skin breakdown, as indicated on the Braden Scale. See Figure 13.2[7] for an image of a client with impaired mobility who developed a DVT.

Strategies to promote client mobility can be divided into two categories: those used when the client is in bed and those used when the client is able to get out of bed. In-bed interventions to enhance mobility include performing repositioning activities, completing range-of-motion exercises, and assisting the client to dangle on the edge of a bed. Out-of-bed interventions to enhance mobility include transferring the client from bed to chair and assisting with ambulation.[9] Unfortunately, ambulation of clients has been identified as the most frequently missed element of inpatient nursing care with rates as high as 76–88% of the time.[10] Before discussing these interventions to promote mobility, let’s review the assessments that a nurse must perform prior to safely implementing mobilization interventions.
Assessing Mobility Status and the Need for Assistance
A client’s mobility status and their need for assistance affect nursing care decisions, such as handling and transferring procedures, ambulation, and implementation of fall precautions. Initial mobility assessments are typically performed on admission to a facility by a physical therapist (PT). See Table 13.2b for an example of common types of assistance required.

In addition to the amount of assistance required, health care providers or physical therapists may determine a client’s weight-bearing status. For example, clients with lower extremity fractures or those recovering from knee or hip replacement often progress through stages of weight-bearing activity. See Table 13.2c for common weight-bearing prescriptions.

In addition to reviewing orders regarding weight-bearing and assistance required, all staff should assess client mobility before and during interventions, such as transferring from surface to surface or during ambulation. Staff may frequently rely on the client’s or a family member’s report on the client’s ability to stand, transfer, and ambulate, but this information can be unreliable. For example, the client may have unrecognized physical deconditioning from the disease or injury that necessitated hospitalization, or they may have developed new cognitive impairments related to the admitting diagnosis or their current medications.[12]
Several objective screening tests, such as the Timed Get Up and Go Test, have traditionally been used by nurses to assess a client’s mobility status. The Timed Get Up and Go Test begins by having the client stand up from an armchair, walk three yards, turn around, walk back to the chair, and sit down. As the client performs these maneuvers, their posture, body alignment, balance, and gait are analyzed. However, this test and other tests do not provide guidance on what the nurse should do if the client can’t maintain seated balance, bear weight, or stand and walk. The Banner Mobility Assessment Tool (BMAT) was developed to provide guidance regarding safe client handling and mobility (SPHM). It is used as a nurse-driven bedside assessment of client mobility and walks the client through a four-step functional task list and identifies the mobility level the client can achieve. It then provides guidance regarding the SPHM technology needed to safely lift, transfer, and mobilize the client.[13] Read additional information about the Banner Mobility Assessment Tool (BMAT) in the following box.

Example of Banner Mobility Assessment Tool In Action[15]
A 65-year-old male was admitted to the hospital late in the evening. He was 6’2” tall and weighed 350 lbs. (158 kg). He needed to have a bowel movement but stated he didn’t want to use a bedpan. The nurse wasn’t comfortable getting him up to use the bathroom because he hadn’t yet been evaluated by physical therapy, and a physical therapist wasn’t available until the following morning. Per agency policy, the nurse assessed the client using the BMAT and found he was able to raise his buttocks off the bed using a bed rail and hold for a count of five. but stated he used a walker at home to ambulate. He was rated at Mobility Level 3, and the nurse transferred him to the toilet using a nonpowered stand aid. Both the client and nurse were relieved and satisfied with the outcome.
Safe Client Handling
Assisting clients with decreased immobility poses an increased risk of injury to health care workers. A focus on safe client handling and mobility in acute and long-term care settings over the past decade has resulted in decreased staff lifting injuries for the first time in 30 years. Nonetheless, nurses still suffer more musculoskeletal disorders from lifting than other employees in the manufacturing and construction industries. Many employers and nurses previously believed that lifting injuries could be prevented by using proper body mechanics, but evidence contradicts this assumption. Body mechanics involves the coordinated effort of muscles, bones, and one’s nervous system to maintain balance, posture, and alignment when moving, transferring, and positioning clients.[16] The National Institute of Occupational Safety and Health (NIOSH) calculates maximum loads for lifting, pushing, pulling, and carrying for all types of employees. For example, a maximum load for employees lifting a box with handles is 50 pounds (23 kg), but this weight is decreased when the lifter has to reach, lift from near the floor, or assume a twisted or awkward position. Because clients don’t come in simple shapes and may sit or lie in awkward positions, move unexpectedly, or have wounds or devices that interfere with lifting, the safe lifting load for clients is less than this maximum 50-pound load. Although using proper body mechanics and good lifting techniques are important, they don’t prevent lifting injuries in these client circumstances[17],[18],[19] Factors that increase risk for lifting injuries in nurses are exertion, frequency, posture, and duration of exposure. Combinations of these factors, such as high exertion while in an awkward posture (for example, holding a client’s leg while bent over and twisted), unpredictable client movements, and extended reaching, intensify the risk.[20]
In 2013 the American Nurses Association (ANA) published Safe Patient Handling and Mobility (SPHM) standards. See the standards in the box below. See other information about safe client handling in the following box.

ANA Standards for Safe Patient Handling and Mobility[21]
Standard 1: Establish a culture of safety. This standard calls for the employer to establish a commitment to a culture of safety. This means prioritizing safety over competing goals in a blame-free environment where individuals can report errors or incidents without fear. The employer is compelled to evaluate systemic issues that contribute to incidents or accidents. The standard also calls for safe staffing levels and improved communication and collaboration. Every organization should have a procedure for nurses to report safety concerns or refuse an assignment due to concern about patients’ or their own safety.
Standard 2: Implement and sustain an SPHM program. This standard outlines SPHM program components, including patient assessment and written guidelines for safe patient handling by staff.
Standard 3: Incorporate ergonomic design principles to provide a safe care environment. This standard is based on the concept of prevention of injuries through ergonomic design that considers the physical layout, work-process flow, and use of technology to reduce exposure to injury or illness.
Standard 4: Select, install, and maintain SPHM technology. This standard provides guidance in selecting, installing, and maintaining SPHM technology.
Standard 5: Establish a system for education, training, and maintaining competence. This standard outlines SPHM training for employees, including the demonstration of competency before using SPHM technology with patients.
Standard 6: Integrate patient-centered SPHM assessment, plan of care, and use of SPHM technology. This standard focuses on the patient’s needs by establishing assessment guidelines and developing an individual plan of care. It outlines the importance of using SPHM technology in a therapeutic manner with the goal of promoting patients’ independence. For example, a patient may need full-body lift technology immediately after surgery, then progress to a sit-to-stand lift for transfers, and then progress to a technology that supports ambulation.
Standard 7: Include SPHM in reasonable accommodation and post-injury return to work. This standard promotes an employee’s return to work after an injury.
Standard 8: Establish a comprehensive evaluation system. The final standard calls for evaluation of outcomes related to an agency’s implementation of a SPHM program with remediation of deficiencies.
Assistive Devices
There are several types of assistive devices that a nurse may incorporate during safe client handling and mobility. An assistive device is an object or piece of equipment designed to help a client with activities of daily living, such as a walker, cane, gait belt, or mechanical lift.[22] Assistive devices include other items described below.
Gait Belts
Gait belts should be used to ensure stability when assisting clients to stand, ambulate, or transfer from bed to chair. A gait belt is a 2-inch-wide (5 mm) belt, with or without handles, that is placed around a client’s waist and fastened with a buckle. The gait belt should be applied on top of clothing or a gown to protect the client’s skin. See Figure 13.3[23] for an image of a gait belt.
Slider Boards
A slider board (also called a transfer board) is used to transfer an immobile client from one surface to another while the client is lying supine (e.g., from a stretcher to hospital bed).[24] See Figure 13.4[25] for an image of a client being transferred by logrolling off a slider board with several assistants.
Sit to Stand Lifts
Sit to stand lifts (also referred to as Sara lifts, lift ups, stand assist, or stand-up lifts) are mobility devices that assist weight-bearing clients who are unable to transition from a sitting position to a standing position using their own strength. They are used to safely transfer clients who have some muscular strength but not enough strength to safely change positions by themselves. Some sit to stand lifts use a mechanized lift whereas others are nonmechanized. See Figure 13.5[26] for an image of a nurse assisting a client to stand with a sit to stand lift.
Mechanical Lifts
A mechanical lift is a hydraulic lift with a sling used to move clients who cannot bear weight or have a medical condition that does not allow them to stand or assist with moving. It can be a portable device or permanently attached to the ceiling. See Figure 13.6[27] for an image of a mechanical lift.
Early Mobility Protocols
Many hospitals use nurse-driven mobility protocols to encourage early mobility of clients in intensive care units and after surgery. The purpose of early mobility protocols is to maintain the client’s baseline mobility and functional capacity, decrease the incidence of delirium, and decrease hospital length of stay. Protocols include a coordinated approach by the multidisciplinary team and may include respiratory therapists, physical therapists, pharmacists, occupational therapists, and the health care provider who focus on getting the client out of bed faster.[28]
When early mobility protocols are in place, nurses use a screening tool to determine whether a client is clinically ready to attempt the protocol. This algorithm begins by reviewing the client’s neurological criteria, such as, does the client open his or her eyes in response to verbal stimulation? If the client meets neurological criteria, they are assessed against additional criteria for respiratory, circulatory, neurological, and other considerations. If the client meets these criteria, a registered nurse may carefully initiate an early mobilization protocol in collaboration with a physical therapist. See Figure 13.7[29] for an example of an early mobilization protocol used for clients in an ICU.[30]

Example of Early Mobilization Protocol[31]
Here is an example of using an early mobilization protocol in an intermediate care unit with patient care technicians (PCT). Three PCTs collaborate with nurses from 7 a.m. to 7 p.m. Each PCT has eight clients and is responsible for mobilizing client during each 12-hour shift. Each patient care technician discusses each client’s level of activity with the RN at the beginning of the shift and determines how many times each client will be mobilized throughout the day. Any concerns that arise during mobilization are shared with the nurse for appropriate follow-up.
Range-of-Motion Exercises
When clients are unable to ambulate or have an injury to specific extremities, range-of-motion (ROM) exercises are often prescribed. ROM exercises facilitate movement of specific joints and promote mobility of the extremities. Because changes in joints can occur after three days of immobility, ROM exercises should be started as soon as possible. There are three types of ROM exercises: passive, active, and active assist. Passive range of motion is movement applied to a joint solely by another person or by a passive motion machine. When passive range of motion is applied, the joint of an individual receiving exercise is completely relaxed while the outside force moves the body part while they are lying in bed. For example, clients who undergo knee replacement surgery may be prescribed a passive motion machine that continuously flexes and extends the client’s knee while lying in bed. See Figure 13.8[32] for an image of a passive motion machine. Active range of motion is movement of a joint by the individual performing the exercise with no outside force aiding in the movement. Active assist range of motion is joint movement with partial assistance from an outside force. For example, during the recovery period after shoulder surgery, a client attends physical therapy and receives 50% assistance in moving the arm with the help of a physical therapy assistant.

Clients may receive temporary ROM exercises due to injury, surgery, or other temporary conditions. These clients are expected to make a full recovery and over time will no longer need ROM to ensure the proper functioning of their joint. Other clients require long-term ROM exercises to prevent contractures that can occur in conditions such as spinal cord injury, stroke, neuromuscular diseases, or traumatic brain injury. A contracture is the lack of full passive range of motion due to joint, muscle, or soft tissue limitations.[33] See Figure 13.9[34] for an image of a severe leg contracture in a client with a terminal neurological condition.
Range-of-motion exercises are prescribed by a physical therapist and can be performed by physical therapy assistants (PTAs), nursing assistants (NAs), patient care technicians (PCTs), and nurses based on agency policy. Guidelines for performing range-of-motion exercises include the following:
- A program of passive stretching should be started as early as possible in the course of neuromuscular disease to prevent contractures and become part of a regular morning and evening routine.
- Proper technique is essential for passive stretching to be effective. With each stretch, the position should be held for a count of 15, and each exercise should be repeated 10 to 15 times during a session (or as prescribed). Stretching should be performed slowly and gently. An overly strenuous stretch may cause discomfort and reduce cooperation.
- Written instructional materials should be provided to the client and family as a supplement to verbal instructions and demonstrations by the physical therapist.

Limb positioning with assistive devices can also be used to prevent contracture formation. The limb should be placed in a resting position that opposes or minimizes flexion.[36] Positioning aids include pillows, foot boots, handrolls, hand-wrist splints, heel or elbow protectors, abduction pillows, or a trapeze bar. See Figure 13.10[37] for an image of a brace used to prevent foot drop in a client with multiple sclerosis. Foot drop is a complication of immobility that results in plantar flexion of the foot, interfering with the ability to complete weight-bearing activities.

Repositioning Clients
Repositioning a bedridden client maintains body alignment and prevents pressure injuries, foot drop, and contractures. Proper positioning also provides comfort for clients who have decreased mobility related to a medical condition or treatment. When repositioning a client in bed, supportive devices such as pillows, rolls, and blankets can aid in providing comfort and safety. There are several potential positions that are determined based on the client’s medical condition, preferences, or treatment related to an illness.[38] It is important to reposition clients appropriately to prevent neurological injuries that can occur, such as if a client is inadvertently positioned on their arm.
Supine Position
In supine positioning, the client lies flat on their back. Pillows or other devices may be used to prevent foot drop. Additional supportive devices, such as pillows under the arms, may be added for comfort. See Figure 13.11[39] for an image of a client in the supine position.[40]
Prone Position
In prone positioning, the client lies on their stomach with their head turned to the side.[41] Pillows may be placed under the lower legs to align the feet. See Figure 13.12[42] for an image of a client in the prone position. Placing clients in the prone position may improve their oxygenation status in certain types of medical disorders, such as COVID-19.[43]
Lateral Position
In lateral positioning, the client lies on one side of their body with the top leg flexed over the bottom leg. This position helps relieve pressure on the coccyx.[44] A pillow may be placed under the top arm for comfort. See Figure 13.13[45] for an image of a client in the lateral position. The lateral position is often used for pregnant women to prevent inferior vena cava compression and enhance blood flow to the fetus.
Sim’s Position
In Sim's positioning, the client is positioned halfway between the supine and prone positions with their legs flexed. A pillow is placed under the top leg. Their arms should be comfortably placed beside them, not underneath.[46] See Figure 13.14[47] for an image of a client in Sim’s position. The Sim’s position is used during some procedures, such as the administration of an enema.
Fowler’s Position
In Fowler’s positioning, the head of bed is placed at a 45- to 90-degree angle. The bed can be positioned to slightly flex the hips to help prevent the client from migrating downwards in bed.[48] See Figure 13.15[49] for an image of a client in Fowler’s position. High Fowler’s position refers to the bed being at a 90-degree angle. The Fowler’s position is used to promote lung expansion and improve a client’s oxygenation. It is also used to prevent aspiration in clients while eating or receiving tube feeding.
Semi-Fowler’s Position
In Semi-Fowler’s positioning, the head of bed is placed at a 30- to 45-degree angle. The client’s hips may or may not be flexed. See Figure 13.16[50] for an image of a client in Semi-Fowler’s position. Semi-Fowler’s position is used for the same purposes as Fowler’s position but is generally better tolerated over long periods of time because there is less pressure on the coccyx area than with Fowler’s and High-Fowler’s position.
Trendelenburg Position
In Trendelenburg positioning, the head of the bed is placed lower than the client’s feet. This position may be used in certain situations to promote venous return to the head and heart, such as during severe hypotension and medical emergencies.[51] See Figure 13.17[52] for an image of Trendelenburg position.
Tripod Position
Clients who are feeling short of breath often naturally assume the tripod position. In the tripod position, the client leans forward while sitting with their elbows on their knees or resting on a table. Clients experiencing breathing difficulties can be placed in this position to enhance lung expansion and air exchange. See Figure 13.18[53] for images of an individual demonstrating breathing difficulty who has assumed the tripod position.
Moving a Client Up in Bed
When moving a client up in bed, first determine the level of assistance needed to provide optimal client care. It is vital to prevent friction and shear when moving a client up in bed to prevent pressure injuries. If a client is unable to assist with repositioning in bed, follow agency policy regarding using lifting devices and mechanical lifts. If the client is able to assist with repositioning and minimal lifting by staff is required, use the following guidelines with assistance from another health care professional to help with the move and prevent injury.[54] See Figure 13.19[55] for an image of moving a client up in bed.
When moving a client up in bed, nurses generally follow these steps[56]:
- Explain to the client what will happen and how the client can help.
- Raise the bed to a safe working height and ensure that the brakes are applied.
- Position the client in the supine position with the bed flat. Place a pillow at the head of the bed and against the headboard to prevent accidentally bumping the client’s head on the headboard.
- Two health care professionals should stand with feet shoulder width apart between the shoulders and hips of the client at the bedside. This keeps the heaviest part of the client closest to the center of gravity of the health care providers. Weight will be shifted from back foot to front foot.
- Fan-fold the draw sheet toward the client with palms facing up. This provides a strong grip to move the client up with the draw sheet.
- Ask the client to tilt their head toward their chest, fold arms across their chest, and bend their knees to assist with the movement. Let the client know when the move will happen. This step prevents injury from occurring to the client and prepares them for the move.
- Tighten your gluteal and abdominal muscles, bend your knees, and keep your back straight and neutral. Face the direction of movement. Proper body mechanics can help prevent back injury when used in appropriate client care situations.
- On the count of three by the lead person, gently slide (not lift) the client up the bed, shifting your weight from the back foot to the front, keeping your back straight and knees slightly bent.
- Replace the pillow under the client’s head, reposition the client in the bed, and cover them with a sheet or blanket to provide comfort.
- Lower the bed, raise the side rails as indicated, and ensure the call light is within reach. Perform hand hygiene.
Assisting Clients to Seated Position
Prior to ambulating, repositioning, or transferring a client from one surface to another (e.g., a bed to a wheelchair), it often necessary to move the client to the side of the bed to avoid straining or excessive reaching by the health care professional. Positioning the client to the side of the bed also allows the health care provider to have the client as close as possible to their center of gravity for optimal balance during client handling.[57]
Clients who have been lying in bed may experience vertigo, a sensation of dizziness as if the room is spinning, or orthostatic hypotension, low blood pressure that occurs when a client changes position from lying to sitting or sitting to standing and causes the client to feel dizzy, faint, or light-headed. Orthostatic hypotension is defined as a drop in systolic blood pressure of 20 mm Hg or more or a drop of diastolic blood pressure of 10 mm Hg or more within three minutes of sitting or standing. For this reason, always begin a transfer or ambulation process by sitting the client on the side of the bed for a few minutes with their legs dangling.[58]
Begin by explaining to the client what will happen and how they can help. Determine if additional assistance or a mechanical lift is needed.[59] See Figure 13.20[60] for images of a nurse assisting a client to a seated position.
When moving a client to a seated position, nurses generally follow these steps[61]:
- Ensure the bed is in a low and locked position.
- Stand facing the head of the bed at a 45-degree angle with your feet apart, with one foot in front of the other. Stand next to the waist of the client.
- Ask the client to turn onto their side, facing you, as they move closer to the edge of the bed.
- Place one hand behind the client’s shoulders, supporting the neck and vertebrae.
- On the count of three, instruct the client to use their elbows to push up against the bed and then grasp the side rail as you support their shoulders as they sit. Shift your weight from the front foot to the back foot as you assist them to sit. Do not allow the client to place their arms around your shoulders because this can lead to serious back or neck injuries.
- As you shift your weight, gently grasp the client’s outer thighs with your other hand and help them slide their feet off the bed to dangle or touch the floor. This step helps the client sit and move their legs off the bed at the same time. As you perform this action, bend your knees and keep your back straight and neutral.
- Assess the client for symptoms of orthostatic hypotension or vertigo. If they are experiencing any dizziness, request them to sit and dangle on the edge of the bed and determine if the symptoms resolve before transferring or ambulating.
Ambulating a Client
Ambulation is the ability of a client to safely walk independently, with assistance from another person, or with an assistive device, such as a cane, walker, or crutches. After a client has been assessed and determined safe to ambulate, determine if assistive devices or the assistance of a second staff member is required. Apply a gait belt snugly over their clothing and around their waist per agency policy if assistance is required. See Figure 13.21[62] for an image of applying a gait belt. The client should be cooperative, able to bear weight on their own, have good trunk control, and be able to transition to a standing position on their own. If these criteria are not met, then mechanical devices, such as a sit to stand lift, should be used to assist a weight-bearing client from a sitting position to a standing position. If a client uses a walker or cane, these assistive devices should be placed near the bed before beginning this procedure.
Nurses generally follow these steps when assisting a seated client to ambulate[63]:
- Assist the client to sit on the side of the bed and assess for symptoms of vertigo or orthostatic hypotension before proceeding. Ensure the client is wearing proper footwear, such as shoes or nonslip socks.
- Stand in front of the client, with your legs on the outside of their legs.
- Grasp each side of the gait belt, while keeping your back straight and knees bent, and then rock your weight backwards while gently steadying the client into a standing position.
- After the client is standing and feels stable, move to their unaffected side and grasp the gait belt in the middle of their back. If needed for stability, place one arm under the client’s arm, gently grasp their forearm, and lock your arm firmly under the client’s axilla. In this position, if the client starts to fall, you can provide support at the client’s shoulder.[64]
- If the client uses a walker or cane, ensure the client is using this device before beginning ambulation.
- Before stepping away from the bed, ask the client if they feel dizzy or light-headed. If they do, sit the client back down on the bed until the symptoms resolve. If the client feels stable, begin walking by matching your steps to the client’s.
- Instruct the client to look ahead and lift each foot off the ground. Walk only as far as the client can tolerate without feeling dizzy or weak.
- Periodically ask them how they are feeling to check for dizziness or weakness.
- In some situations of early ambulation, it is helpful for a second staff member to follow behind the client with a wheeled walker or wheelchair in case the client needs to sit while walking. See Figure 13.22[65] for an image of a nurse assisting the client to stand.
To assist the client back into the bed or a chair, have them stand with the back of their knees touching the bed or chair. Grasp the gait belt and assist them as they lower into a sitting position, keeping your back straight and knees bent. Remove the gait belt. If the client is returning to bed, place the bed in the lowest position, raise the side rails as indicated, and ensure the call light is within reach. Cover the client with a sheet or blanket to provide comfort. Document the distance of ambulation and the client’s tolerance of ambulation.
Transfer From Bed to Chair or Wheelchair
Clients often require assistance when moving from a bed to a chair or wheelchair. A client must be cooperative and predictable, able to bear weight on both legs, and able to take small steps and pivot to safely transfer with a one-person assist. If any of these criteria are not met, a two-person transfer, or mechanical lift is recommended. Always complete a mobility assessment and check the provider’s or physical therapist’s orders prior to transferring clients.[66]
Nurses generally follow these steps when moving a client from a bed or chair to a wheelchair[67]:
- Begin by explaining to the client what will happen during the transfer and how they can help. Be sure proper footwear is in place. Lower the bed to a 45-degree angle. Place the wheelchair next to the bed and apply the wheelchair brakes. If the client has weakness on one side, place the wheelchair on their strong side.
- Assist the client to a seated position on the side of the bed with their feet on the floor. (See the previous section on how to assist a client to a seated position.)
- Apply the gait belt snugly around their waist.
- Place your legs on the outside of their legs. Ask them to place their hands on your waist as they raise themselves into a standing position. Do not lift the client. If additional assistance is required, obtain a mechanical lift, such as a sit to stand device. Do not allow them to put their arms around your neck because this can cause neck or back injury.
- Stay close to the client during the transfer to keep the client’s weight close to your center of gravity.
- Once standing, ask the client to pivot and then take a few steps back until they can feel the wheelchair on the back of their legs.
- Have the client grasp the arm of the wheelchair and lean forward slightly.
- Assist the client to lower themselves, while shifting your weight from your back leg to the front leg with your knees bent, trunk straight, and elbows slightly bent. Allow the client to slowly lower themselves into the wheelchair using the armrests for support.

Lowering A Client to the Floor
A client may begin to fall while ambulating or while being transferred from one surface to another. If a client begins to fall from a standing position, do not attempt to stop the fall or catch the client because this can cause back injury. Instead, try to control their fall by lowering them to the floor.[70]
If a client starts to fall and you are close by, move behind the client and take one step back. Support the client around the waist or hip area or grab the gait belt. Bend one leg and place it between the clients legs. Slowly slide the client down your leg, lowering yourself to the floor at the same time. Always protect their head first. Once the client is on the floor, assess the client for injuries prior to moving them. Assess the client’s need for assistance to get off the floor. If the client is unable to get up off the floor, use a mechanical lift. Complete an incident report and follow up according to the client’s condition and agency policy. See Figure 13.24[71] for images of lowering a client to the floor.
Preventing Falls
Falls are a major safety concern in health care. Nurses are responsible for identifying, managing, and eliminating potential fall hazards for clients. All client-handling activities (positioning, transfers, and ambulation) pose a risk to both clients and health care professionals. Older adults are often at increased risk for falls due to impaired mental status, decreased strength, impaired balance and mobility, and decreased sensory perception. Clients may also be at risk for falls due to gait problems, cognitive ability, visual problems, urinary frequency, generalized weakness, cognitive impairments, or medications that may cause hypotension or drowsiness.[72] Falls can cause head injuries, fractures, lacerations, and other injuries.
Fall prevention is key. If a client begins to feel dizzy while ambulating or transferring, assist them to sit on a chair or on the floor to avoid a fall. The head is the most important part of the body, so protect it as much as possible. In the event of a fall, seek help and stay with the client until assistance arrives. Follow agency policy for reporting, assessing, and documenting. After a fall, always assess a client for injuries prior to moving them. If the client remains weak or dizzy, do not attempt to ambulate them, but instead, ask for assistance to transfer them to a chair or bed.[73]
All clients should be assessed for risk factors for falls and necessary fall precautions implemented per agency policy. Read more information about preventing falls in the “Safety” chapter.

- Rub hands palm to palm
- Back of right and left hand (fingers interlaced)

Head-to-Toe Assessment Checklist
This checklist is intended as a guide for a routine, general, daily assessment performed by an entry-level nurse during inpatient care. Students should use a systematic approach and include these components in their assessment and documentation. Assessment techniques should be modified according to life span considerations. Focused assessments should be performed for abnormal findings and according to specialty unit guidelines. Unanticipated findings should be reported per agency protocol with emergency assistance obtained as indicated.
- Gather supplies: stethoscope, penlight, watch with second hand, gloves, hand sanitizer, and wound measurement tool.
- Perform hand hygiene before providing care and clean stethoscope.
- Perform safety steps:
- Knock before entering the room.
- Check the room for transmission-based precautions.
- Introduce yourself, your role, the purpose of your visit, and an estimate of the time it will take.
- Explain the process to the patient and ask if they have any questions.
- Be organized and systematic.
- Use appropriate listening and questioning skills.
- Listen and attend to patient cues.
- Ensure the patient’s privacy and dignity.
- Identify the patient using two patient identifiers (e.g., name and date of birth).
- Perform a primary survey to ensure medical stability. Obtain emergency assistance if needed.

- Evaluate for the presence of pain or other type of discomfort. If pain or discomfort is present, perform comprehensive pain assessment using PQRSTU.
- Perform a neuromuscular assessment:
- Perform a subjective assessment. Ask if headache, dizziness, weakness, numbness, tingling, or tremors are present. Inquire if the patient has experienced loss of balance, decreased coordination, previous falls, or difficulty swallowing. Be aware of previously diagnosed neuromuscular conditions and currently prescribed medications and how these impact your assessment findings.

- Ask if they are having any problems with their teeth or gums, and if this has impacted their ability to eat.
- Ask if they use glasses, hearing aids, or dentures.
- Ask if they have any difficulty seeing or blurred vision.
- Ask if they have trouble hearing or experience ringing in their ears.
- Inspect the external eye and the external ear. Inspect the oral cavity for lesions, tongue position, moisture, and oral health. Ask the patient to swallow their saliva and note any difficulty swallowing.
- Palpate the lymph nodes (per agency policy).
- Perform a subjective assessment. Be aware of previously diagnosed head, neck, eye, or ear conditions and associated medications and how these impact your assessment findings.
- Perform a cardiovascular system assessment:
- Perform a subjective assessment. Ask if they are having chest pain, shortness of breath, edema, palpitations, calf pain, or pain in their feet or lower legs when exercising. Be aware of previously diagnosed cardiovascular conditions and currently prescribed medications and how these impact your assessment findings.
- Inspect:
- The face, lips, and extremities for pallor or cyanosis.
- The neck for JVD in upright position or with head of bed at 30-45-degree angle.
- The bilateral upper and lower extremities for color, warmth, and sensation.
- The lower extremities for hair distribution, edema, and signs of deep vein thrombosis (DVT).
- Palpate:
- Palpate and compare the radial, brachial, dorsalis pedis, and posterior tibial pulses bilaterally. Note the presence and amplitude of pulses.
- Palate the nail beds for capillary refill.
- Auscultate:
- Auscultate with both the bell and the diaphragm of the stethoscope over the five auscultation areas of the heart. Note the rate and rhythm. Identify S1 and S2 and any unexpected findings (i.e., extra sounds or irregular rhythm).
- Measure the apical pulse for one minute.
- Perform a respiratory assessment:
- Perform a subjective assessment. Ask if they have shortness of breath or a cough. Ask if the cough is dry or productive. Ask if they smoke, and if so, what products, how many a day, and if they are interested in quitting. Be aware of previously diagnosed respiratory conditions and currently prescribed medications or treatments and how these impact your assessment findings.
- Inspect:
- Level of consciousness and for signs of irritability, restlessness, anxiety, or confusion.
- Breathing pattern, including rate, rhythm, effort, and depth of breathing. Note signs of difficulty breathing such as nasal flaring, use of accessory muscles, or pursed-lip breathing.
- Skin color of lips, face, hands and feet for cyanosis and pallor.
- Trachea (midline).
- Symmetrical chest movement.
- Auscultate lung sounds using stethoscope directly on the skin over anterior and posterior auscultation areas. Compare sounds from side to side and note any adventitious sounds such as rhonchi, crackles, wheezing, stridor, or pleural rub.
- If oxygen equipment is prescribed:
- Note if the patient is using oxygenation devices during the exam or on room air.
- If the patient is using an oxygenation device, document the name of device and current flow rate and/or fraction of inspired oxygen (FiO2).
- Inspect for signs of skin breakdown due to the use of oxygenation devices.
- If a tracheostomy is present, document the condition of the tracheostomy site and characteristics of sputum present.
- Perform an abdominal assessment:
- Perform a subjective assessment:
- Ask if the patient is having any abdominal pain, cramping, nausea, vomiting, constipation, loss of appetite, or difficulty swallowing. Inquire about the date of the last bowel movement, if there have been any changes in the pattern or consistency of the stool, and if any blood is present or dark stool. Be aware of previously diagnosed gastrointestinal or genitourinary conditions and currently prescribed medications and how these impact your assessment findings.
- Ask if the patient has pain or problems with urination or leakage of urine.
- Inspect the general contour and symmetry of the abdomen and for distension.
- Auscultate for bowel sounds over four quadrants for one minute, note any hypoactive, high-pitched sounds.
- Palpate lightly for tenderness and masses.
- Analyze weight trend and 24-hour input and output, as appropriate for patient status.
- If enteral tube is present, assess tube insertion site, tube placement, and amount of enteral feeding/fluids administered during your shift per agency policy.
- If an indwelling urinary catheter is present, assess urine output and urine characteristics. Document continued need for indwelling catheter per agency policy.
- If an ostomy is present, document the condition of stoma and peristomal skin. Document amount and characteristics of output during your shift.
- Perform a subjective assessment:
- Perform an integumentary assessment:
- Perform a subjective assessment. Ask if the patient has any skin concerns such as itching, rashes, or an unusual mole or lump. Be aware of previously diagnosed integumentary conditions and currently prescribed medications or treatments and how these impact your assessment findings.
- Inspect:
- Assess overall skin color and note pallor, cyanosis, jaundice, erythema, bruising, moisture, and turgor.
- If an intravenous site is present, assess the insertion site for redness, warmth, tenderness, or induration. If intravenous fluids and/or medications are infusing, document the type and amount of fluids during your shift per agency policy.
- Assess for skin breakdown in pressure points (behind ears, occipital area, elbows, sacrum, and heels).
- If a pressure injury is present, stage from 1 to 4.
- If a wound is present, perform a wound assessment.
- Palpate for temperature, moisture, and texture. If erythema or rashes are present, assess for blanching. If edema is present, document the depth of indentation and the time it takes to rebound to original position and grade on a scale from 1 to 4.
- When the assessment is completed, assist the patient back to a comfortable position. Thank them and ask if anything is needed before you leave the room.
- Ensure safety measures when leaving room:
- CALL LIGHT: Within reach
- BED: Low and locked (in lowest position and brakes on)
- SIDE RAILS: Secured
- TABLE: Within reach
- ROOM: Risk-free for falls (scan room and clear any obstacles)
- Remove any PPE before leaving the room. Perform hand hygiene and clean stethoscope.
- Document assessment findings and report unanticipated findings according to agency policy.

Abduction: Joint movement away from the midline of the body. (Chapter 13.2)
Accessory muscles: Muscles other than the diaphragm and intercostal muscles that may be used for labored breathing. (Chapter 10.3)
Accommodation: The ability of the eye to adjust from near vision to far vision. Pupils constrict at near vision and dilate at far vision. (Chapter 6.5)
Acne: A skin disturbance that typically occurs on areas of the skin that are rich in sebaceous glands, such as the face and back. (Chapter 14.3)
Active range of motion: The degree of movement a patient can voluntarily achieve in a joint without assistance. (Chapter 13.4)

Airborne precautions: Infection prevention and control interventions to be used in addition to standard precautions for diseases spread by airborne transmission, such as measles and tuberculosis. (Chapter 4.2)
Ampules: Small glass containers of liquid medication ranging from 1 mL to 10 mL sizes. (Chapter 18.2)
Angiogenesis: The development of new capillaries in a wound base. (Chapter 20.2)
Anosmia: Partial or complete loss of smell. This symptom can be related to underlying cranial nerve dysfunction or other nonpathological causes such as a common cold. (Chapter 6.5)
Apnea: Absence of respirations. (Chapter 10.3)
Apocrine sweat gland: Sweat glands associated with hair follicles in densely hairy areas that release organic compounds subject to bacterial decomposition causing odor. (Chapter 14.2)
Around the Clock (ATC) order: An order that reflects that medication should be administered at regular time intervals, such as every six hours, to maintain consistent levels of the drug in the patient’s bloodstream. (Chapter 15.2)
Arterial blood gas (ABG): A sample of arterial blood that measures the oxygen, carbon dioxide, and bicarbonate levels. (Chapter 11.2)
Arterial blood sampling: Blood is obtained via puncture into an artery by specially trained registered nurses and other health care personnel, such as respiratory therapists, physicians, nurse practitioners, and physician assistants. (Chapter 23.2)
Arterial ulcers: Ulcers caused by lack of blood flow and oxygenation to tissues and typically occur in the distal areas of the body such as the feet, heels, and toes. (Chapter 20.2)
Arthroplasty: Joint replacement surgery. (Chapter 13.3)
Arthroscopic surgery: A surgical procedure involving a small incision and the insertion of an arthroscope, a pencil-thin instrument that allows for visualization of the joint interior. Small surgical instruments are inserted via additional incisions to remove or repair ligaments and other joint structures. (Chapter 13.3)
Articular cartilage: Smooth, white tissue that covers the ends of bones where they come together at joints, allowing them to glide over each other with very little friction. Articular cartilage can be damaged by injury or normal wear and tear. (Chapter 13.2)
Asepsis: A state of being free of disease-causing microorganisms. (Chapter 4.3)
Aseptic non-touch technique: A standardized technique, supported by evidence, to maintain asepsis and standardize practice. (Chapter 4.3)
Aseptic technique (medical asepsis): The purposeful reduction of pathogen numbers while preventing microorganism transfer from one person or object to another. This technique is commonly used to perform invasive procedures, such as IV starts or urinary catheterization. (Chapter 4.3)
Aspiration pneumonia: A type of lung infection caused by material from the stomach or mouth inadvertently entering the lungs that can be life-threatening. (Chapter 7.3, Chapter 12.3)
Atelectasis: Alveoli or an entire lung is collapsed, allowing no air movement. (Chapter 10.3)
Atrial fibrillation: An irregular heartbeat that is often fast and increases the risk of heart attack or stroke. (Chapter 9.3)
Auricle: The large, fleshy structure of the ear on the lateral aspect of the head. (Chapter 8.2)
Auscultation: Listening to sounds, such as heart, lung, and bowel sounds, created by organs using a stethoscope. (Chapter 1.1)
B
Babinski response: A reflex demonstrated as fanning of 4 toes with great toe bending towards top of foot when object slid along sole of foot. Normal in children under 2. Older than 2 through adulthood, all five toes should curl downward. (Chapter 6.9)
Barrel-chested: An equal AP-to-transverse diameter that often occurs in patients with COPD due to hyperinflation of the lungs. (Chapter 10.3)
Belief: Something accepted as true with a sense of certainty. (Chapter 2.8)
Bladder scan: A bedside test using a noninvasive tool used to measure the volume of urine in the bladder. (Chapter 21.6)
Blanching: To make white or pale by applying pressure. (Chapter 9.3, Chapter 14.4)
Bleb: A small, raised circle that appears after administration of an intradermal medication indicating correct placement into the dermis. (Chapter 18.4)
Body image: A mental picture of one’s body related to appearance and function. (Chapter 2.8)
Body Mass Index (BMI): A standardized reference range to gauge a patient’s weight status. (Chapter 1.4)
Borborygmus: Hyperperistalsis, often referred to as “stomach growling.” (Chapter 12.3)
Bradypnea: Decreased respiratory rate or slow breath less than normal range according to the patient’s age. (Chapter 10.3)
Broca’s area: An area located in the frontal lobe that is responsible for the production of language and controlling movements responsible for speech. (Chapter 6.2)
Bronchial breath sounds: High-pitched hollow sounds heard over trachea and the larynx. (Chapter 10.3)
Bronchovesicular sounds: Mixture of low- and high-pitched sounds heard over major bronchi. (Chapter 10.3)
Bruit: A swishing sound heard upon auscultation. (Chapter 6.10, Chapter 9.3)
C
Candidiasis: A fungal infection often referred to as “thrush” when it occurs in the oral cavity in children. (Chapter 7.3)
Capillary blood testing: Blood collected from capillaries located near the surface of the skin. (Chapter 23.2)
Capillary refill: The time it takes for color to return after pressure is applied to tissue causing blanching. (Chapter 9.3)
Care partners: Family and friends who are involved in helping to care for the patient. (Chapter 2.2)
Catheter embolism: Occurs when a small part of the cannula breaks off and flows into the vascular system. (Chapter 23.4)
CAUTI: Catheter-associated urinary tract infection. (Chapter 21.3)
Central nervous system: The part of the nervous system that includes the brain (the interpretation center) and the spinal cord (the transmission pathway). (Chapter 6.2)
Central venous access device (CAVD): A type of vascular access that involves the insertion of a catheter into a large vein in the arm, neck, chest, or groin. (Chapter 23.2)
Cerebellum: The part of the brain that coordinates skeletal and smooth muscle movement and maintains equilibrium and balance. (Chapter 6.2)
Cerebral cortex: The cerebrum is covered by a wrinkled outer layer of gray matter. (Chapter 6.2)
Cerumen impaction: A buildup of earwax causing occlusion of the ear canal. (Chapter 8.2)
Chief complaint: The reason a patient is seeking health care during a visit to a clinic or on admission to a health care facility. (Chapter 2.5)
Cleft lip: A birth defect caused by a partial or complete failure of the right and left portions of the upper lip to fuse together, leaving a gap in the lip. (Chapter 7.3)
Cleft palate: A birth defect caused when two halves of the hard palate fail to completely come together and fuse at the midline, leaving a gap between them, and making it very difficult for an infant to generate the suckling needed for nursing. (Chapter 7.3)
Click: Clicking sound heard on auscultation of the precordium; often heard in patients with heart valve abnormalities. (Chapter 9.3)
Clubbing: A change in the configuration where the tips of the nails curve around the fingertips, usually caused by chronic low levels of oxygen in the blood. (Chapter 10.3, Chapter 11.2)
Clubfoot: A congenital condition that causes the foot and lower leg to turn inward and downward. (Chapter 13.3)
Colostomy: The colon is attached to a stoma to bypass the rectum and the anus. (Chapter 21.7)
Comatose: A decreased level of consciousness with a score of less than 8 on the Glasgow Coma Scale. (Chapter 6.4)
Concussion: A type of traumatic brain injury caused by a bump, blow, or jolt to the head or by a hit to the body that causes the head and brain to move rapidly back and forth. This sudden movement can cause the brain to bounce around or twist in the skull, creating chemical changes in the brain and damaging brain cells. (Chapter 7.3)
Conductive hearing loss: Hearing loss that occurs when something in the external or middle ear is obstructing the transmission of sound. (Chapter 8.2)
Congenital condition: A condition present at birth. (Chapter 13.3)
Conjunctiva: Inner surface of the eyelid. (Chapter 8.2)
Conjunctivitis: A viral or bacterial infection in the eye causing swelling and redness in the conjunctiva and sclera. (Chapter 8.2)
Contact precautions: Infection prevention and control interventions to be used, in addition to standard precautions, for diseases spread by contact with the patient, their body fluids, or their surroundings, such as C-diff, MRSA, VRE, and RSV. (Chapter 4.2)
Contracture: A fixed or permanent tightening of muscles, tendons, ligaments, or the skin that prevents normal movement of the body part. (Chapter 13.3)
Convergence: The action of both eyes moving inward as they focus on a close object using near vision. (Chapter 6.5)
Cornea: The transparent front part of the eye that covers the iris, pupil, and anterior chamber. (Chapter 8.2)
Coude catheter: A catheter specifically designed to maneuver around obstructions or blockages in the urethra such as with enlarged prostate glands in males. Coude originates from the French word that means “bend.” (Chapter 21.2)
Crackles: Also referred to as “rales”; sound like popping or crackling noises during inspiration. Associated with inflammation and fluid accumulation in the alveoli. (Chapter 10.3)
Cranium: Eight bones that protect the brain in the cranial cavity. (Chapter 7.2)
Crepitus: A crackling, popping noise heard on joint movement. It is considered normal when it is not associated with pain. (Chapter 10.3, Chapter 13.2)
Cultural safety: The creation of safe spaces for patients to interact with health professionals without judgment, racial reductionism, racialization, or discrimination. (Chapter 1.2, Chapter 2.2)
Cyanosis: Bluish discoloration of the skin, lips, and nail beds. It is an indication of decreased perfusion and oxygenation. (Chapter 9.3, Chapter 10.3, Chapter 11.2, Chapter 14.4)
D
Debridement: The removal of nonviable tissue in a wound. (Chapter 20.4)
Deep Vein Thrombosis (DVT): A blood clot that forms in a vein deep in the body. (Chapter 9.3)
Dehiscence: The separation of the edges of a surgical wound. (Chapter 20.2)
Deltoid: Commonly used for intramuscular vaccinations in adults because it has a triangular shape and is easy to locate and access. The injection site is in the middle of the deltoid muscle, about 1 to 2 inches below the acromion process. (Chapter 18.6)
Dermatome: An area of the skin that is supplied by a single spinal nerve. (Chapter 6.6)
Dermis: The inner layer of skin with connective tissue, blood vessels, sweat glands, nerves, hair follicles, and other structures. (Chapter 14.2)
Developmental stages: A person’s life span can be classified into nine categories of development, including Prenatal Development, Infancy and Toddlerhood, Early Childhood, Middle Childhood, Adolescence, Early Adulthood, Middle Adulthood, Late Adulthood, and Death and Dying. (Chapter 1.2)
Diabetic ulcers: Ulcers that typically develop on the plantar aspect of the feet and toes of patients with diabetes due to lack of sensation of pressure or injury. (Chapter 20.2)
Diaphoretic: Excessive, abnormal sweating. (Chapter 14.4)
Diastole: The phase between each contraction of the heart when the ventricles are filling with blood. (Chapter 3.2)
Diastolic blood pressure: The resting pressure of blood on the arteries between each cardiac contraction. (Chapter 3.2)
Dimensional analysis: Dimensional analysis is a problem-solving technique where measurements are converted to a different (but equivalent) unit of measure by multiplying with a fractional form of 1 to obtain a desired unit of administration. (Chapter 5.6)
Diplopia: Double vision (i.e., seeing two images of a single object). (Chapter 6.5)
Dislocation: A joint injury that forces the ends of bones out of position; often caused by a fall or a blow to the joint. (Chapter 13.3)
Distention: An expansion of the abdomen caused by the accumulation of air or fluid. Patients often report “feeling bloated.” (Chapter 12.3)
Doff: To take off or remove personal protective equipment, such as gloves or a gown. (Chapter 4.2)
Don: To put on equipment for personal protection, such as gloves or a gown. (Chapter 4.2)
Drop factor: The number of drops in one mL of solution when fluids or medications are administered using gravity IV tubing. (Chapter 5.14)
Droplet precautions: Infection prevention and control interventions to be used in addition to standard precautions; used for diseases spread by large respiratory droplets such as influenza, COVID-19, or pertussis. (Chapter 4.2)
Dry powder inhaler (DPI): An inhaler with medication provided in a powder form that is inhaled from the mouth into the lungs using a quick breath to activate the medication and move it into the lungs. An example of a DPI is tiotropium (Spiriva). (Chapter 16.2)
Ductus arteriosus: Shunt that connects the pulmonary artery and aorta in the developing fetus. (Chapter 9.3)
Dysphagia: Difficulty swallowing. (Chapter 6.10, Chapter 7.2, Chapter 7.3, Chapter 12.3, Chapter 15.2)
Dyspnea: A feeling of shortness of breath. (Chapter 9.3, Chapter 10.3, Chapter 11.2)
Dysuria: Painful urination. (Chapter 2.8, Chapter 12.3)
E
Ecchymosis: Bruising that occurs when small veins and capillaries under the skin break. (Chapter 14.4, Chapter 20.2)
Eccrine sweat gland: Sweat gland that produces hypotonic sweat for thermoregulation. (Chapter 14.2)
Edema: Swelling in tissues caused by fluid retention. (Chapter 9.3, Chapter 20.2)
Elimination: Refers to the removal of waste products through the urine and stool. (Chapter 2.8)
eMAR: Electronic medication administration record contained in a patient’s electronic chart. (Chapter 15.2)
Enhanced barrier precautions: An infection control intervention designed to reduce transmission of multidrug-resistant organisms (MDRO’S) in nursing homes. (Chapter 4.2)
Enteral medications: Medications that are administered directly into the gastrointestinal tract orally, rectally, or through a tube such as a nasogastric (NG) tube, nasointestinal (NI) tube, or percutaneous endoscopic gastrostomy (PEG) tube. (Chapter 15.1)
Enteral nutrition (EN): Nutrition provided directly into the gastrointestinal (GI) tract through an enteral tube that bypasses the oral cavity. (Chapter 17.2)
Epidermis: The thin, uppermost layer of skin. (Chapter 14.2)
Epiglottis: A flexible piece of cartilage that covers the opening of the trachea during swallowing to prevent ingested material from entering the trachea. (Chapter 7.2)
Epistaxis: Nosebleed. (Chapter 7.3)
Epithelialization: The development of new epidermis and granulation tissue. (Chapter 20.2)
Equivalency: Two values or quantities that are the same amount. For example, one cup is equivalent to eight ounces. (Chapter 5.4)
Erythema: Redness. (Chapter 14.4, Chapter 20.2, Chapter 20.3)
Eschar: Dark brown/black, dry, thick, and leathery dead tissue in a wound base that must be removed for healing to occur. (Chapter 20.2)
Eustachian tube: The tube connecting the middle ear to the pharynx that helps equilibrate air pressure across the tympanic membrane. (Chapter 8.2)
Extension: Joint movement causing the straightening of limbs (increase in angle) at a joint. (Chapter 13.2)
Extraocular muscles: Six muscles that control the movement of the eye within the orbit. Extraocular muscles are innervated by three cranial nerves, the abducens nerve, the trochlear nerve, and the oculomotor nerve. (Chapter 8.2)
Extravasation: The infiltration of damaging intravenous medications, such as chemotherapy, into the extravascular tissue around the site of infusion, causing tissue injury and possible necrosis. (Chapter 23.4)
Exudate: Fluid that oozes out of a wound; also commonly called pus. (Chapter 20.2)
F
Facial drooping: An asymmetrical facial expression that occurs due to damage of the nerve innervating a particular part of the face. (Chapter 7.2)
Family dynamics: Patterns of interactions between family members that influence family structure, hierarchy, roles, values, and behaviors. (Chapter 1.4)
Fenestrated cannula: Type of tracheostomy tube that contains holes so the patient can speak if the cuff is deflated and the inner cannula is removed. (Chapter 22.2)
15-15 Rule: A rule in an agency’s hypoglycemia protocols that includes providing 15 grams of carbohydrate, and then repeating the blood glucose reading in 15 minutes, and then repeating as needed until the patient’s blood glucose reading is above 70. (Chapter 19.2)
FiO2: Fraction of inspired oxygen (i.e., the concentration of oxygen inhaled). Room air contains 21% oxygen levels, and oxygenation devices can increase the inhaled concentration of oxygen up to 100%. However, FiO2 levels should be decreased as soon as feasible to do so to prevent lung injury. (Chapter 11.2)
First-degree burn: A superficial burn that affects only the epidermis. (Chapter 14.3)
Five moments of hand hygiene: Hand hygiene should be performed during the five moments of patient care: immediately before touching a patient; before performing an aseptic task or handling invasive devices; before moving from a soiled body site to a clean body site on a patient; after touching a patient or their immediate environment; after contact with blood, body fluids, or contaminated surfaces (with or without glove use); and immediately after glove removal. (Chapter 4.2)
Flange: The end of the tracheostomy tube that is placed securely against the patient’s neck. (Chapter 22.2)
Flexion: Joint movement causing the bending of the limbs (reduction of angle) at a joint. (Chapter 13.2)
Fluid volume overload (hypervolemia): A condition when there is too much fluid in the blood. Patients may present with shortness of breath, edema to the extremities, and weight gain. (Chapter 23.2)
Foot drop: The inability to raise the front part of the foot due to weakness or paralysis of the muscles that lift the foot. (Chapter 13.3)

Gastric residual volume (GRV): Stomach contents aspirated with a 60-mL syringe, typically performed for patients receiving enteral feeding to assess aspiration risk with associated interventions such as slowing or stopping tube feeding. GRVs in the range of 200–500 mL should raise concern and lead to the implementation of measures to reduce risk of aspiration. (Chapter 17.2)
Gauge: Refers to the diameter of a needle. Gauges can vary from very small diameter (25 to 29 gauge) to large diameter (18 to 22 gauge). (Chapter 18.2)
Gender expression: Characteristics in appearance, personality, and behavior, culturally defined as masculine or feminine. (Chapter 2.8)
Gender identity: One’s basic sense of being male, female, or other gender. (Chapter 2.8)
General survey assessment: A component of a patient assessment that observes the entire patient as a whole. Observation includes using all five senses to gather cues that provide a guideline for additional focused assessments in areas of concern. (Chapter 1.1)
Goiter: An abnormal enlargement of the thyroid gland that can occur with hypothyroidism or hyperthyroidism. (Chapter 7.3)
Gout: A type of arthritis that causes swollen, red, hot, and stiff joints due to the buildup of uric acid, commonly starting in the big toe. (Chapter 13.3)
Granulation tissue: New connective tissue in a wound base with fragile, thin-walled capillaries that must be protected. (Chapter 20.2)
Guarding: Voluntary contraction of abdominal wall musculature; may be related to fear, anxiety, or presence of cold hands. (Chapter 12.3)
H
Hand hygiene: A way of cleaning one’s hands to substantially reduce the number of pathogens and other contaminants (e.g., dirt, body fluids, chemicals, or other unwanted substances) to prevent disease transmission or integumentary harm, typically using soap, water, and friction. An alcohol-based hand rub solution may be appropriate hand hygiene for hands not visibly soiled. (Chapter 4.1)
HCO3: Bicarbonate level reflected in arterial blood gas results. Normal range is 22-26 meq/L. (Chapter 11.2)
Healthcare-Associated Infections (HAIs): Unintended infections caused by care received in a health care setting. (Chapter 4.1)
Health history: The process of using directed interview questions to obtain symptoms and perceptions about a patient’s illness or life condition. The purpose of obtaining a health history is to gather subjective data from the patient and/or the patient’s family so that the health care team and the patient can collaboratively create a plan that will promote health, address acute health problems, and minimize chronic health conditions. (Chapter 2.1)
Heave or lift: Palpable lifting sensation under the sternum and anterior chest wall to the left of the sternum; it suggests severe right ventricular hypertrophy. (Chapter 9.3)
Hematemesis: Blood-tinged mucus secretions from the lungs. (Chapter 12.3)
Hematochezia: Passage of bloody stool. (Chapter 12.3)
Hematoma: An area of blood that collects outside of larger blood vessels. (Chapter 7.2, Chapter 20.2)
Hemoptysis: Blood-tinged mucus secretions from the lungs. (Chapter 10.3)
Hemosiderin staining: Dark-colored discoloration of the lower legs due to blood pooling. (Chapter 20.2)
Hemostasis phase: The first phase of wound healing that occurs immediately after skin injury. Blood vessels constrict and clotting factors are activated. (Chapter 20.2)
Hyperactive bowel sounds: Increased peristaltic activity; may be related to diarrhea, obstruction, or digestion of a meal. (Chapter 12.3)
Hypercapnia: Elevated carbon dioxide levels in the blood, indicated by PaCO2 level greater than 45 in an ABG test. (Chapter 10.3, Chapter 11.2)
Hyperglycemia: Elevated blood glucose reading with associated signs and symptoms such as frequent urination and increased thirst. (Chapter 19.2)
Hypertension: Elevated blood pressure over 130/80 mmHg in an adult. (Chapter 3.2)
Hypertonic solutions: Solutions that have a higher concentration of dissolved particles than blood. (Chapter 23.2)
Hypoactive bowel sounds: Decreased peristaltic activity; may be related to constipation following abdominal surgery or with an ileus. (Chapter 12.3)
Hypodermis: The layer of skin beneath the dermis composed of connective tissue and used for fat storage. (Chapter 14.2)
Hypoglycemia: A blood glucose reading less than 70 associated with symptoms such as irritability, shakiness, hunger, weakness, or confusion. If not rapidly treated, hypoglycemia can cause seizures and a coma. (Chapter 19.2)
Hypotension: Decreased blood pressure less than 90/60 mmHg in an adult. (Chapter 3.2)
Hypothalamus: The autonomic control center of the brain that controls functions such as blood pressure, heart rate, digestive movement, and pain perception. (Chapter 6.2)
Hypotonic solutions: Solutions that have a lower concentration of dissolved solutes than blood. (Chapter 23.2)
Hypoxemia: Decreased dissolved oxygen in the arterial blood, indicated by a PaO2 level less than 80 mmHg in an ABG test. (Chapter 10.3, Chapter 11.2)
Hypoxia: A reduced level of tissue oxygenation. (Chapter 11.2)

Ileostomy: The lower end of the small intestine (ileum) is attached to a stoma to bypass the colon, rectum, and anus. (Chapter 21.7)
Incident report: A report submitted per agency policy used to document the events surrounding a medication error. (Chapter 15.2)
Induration: Area of hardened tissue. (Chapter 20.3)
Indwelling catheter: A device often referred to as a “Foley catheter” that is inserted into the neck of the bladder and remains in place for continual collection of urine into a collection bag. (Chapter 21.2)
Infiltration: Infiltration occurs when the tip of the IV catheter slips out of the vein, the catheter passes through the wall of the vein, or the blood vessel wall allows part of the fluid to infuse into the surrounding tissue, resulting in the leakage of IV fluids into the surrounding tissue. (Chapter 23.4)
Inflammatory phase: The second phase of wound healing when vasodilation occurs so that white blood cells in the bloodstream can move into the wound to start cleaning the wound bed. (Chapter 20.2)
Inner cannula: The cannula inside the outer cannula that is removed during tracheostomy care by the nurse. Inner cannulas can be disposable or reusable with appropriate cleaning. (Chapter 22.2)
Inspection: The observation of a patient’s anatomical structures. (Chapter 1.1)
Instrumental activities of daily living: Complex daily tasks that allow patients to function independently such as managing finances, paying bills, purchasing and preparing meals, managing one’s household, taking medications, and facilitating transportation. (Chapter 2.8)
Intermittent catheterization: The insertion and removal of a straight catheter for relief of urinary retention. (Chapter 21.2)
Intradermal injection: Medication administered in the dermis just below the epidermis. (Chapter 18.1)
Intramuscular injection: Medication administered into a muscle. (Chapter 18.1)
Intravenous injection: Medication administered directly into the bloodstream. (Chapter 18.1)
Intravenous therapy (IV therapy): Involves the administration of substances such as fluids, electrolytes, blood products, nutrition, or medications directly into a client's vein. (Chapter 23.2)
Inunction: A medication that is massaged or rubbed into the skin. (Chapter 16.2)
Involuntary guarding: The reflexive contraction of overlying abdominal muscles as the result of peritoneal inflammation. (Chapter 12.3)
Iris: Colored part of the eye. (Chapter 8.2)
Isotonic solutions: IV fluids that have a similar concentration of dissolved particles as found in the blood. (Chapter 23.2)

Jaundice: A yellowing of the skin or sclera caused by underlying medical conditions. (Chapter 14.3, Chapter 14.4)
Joints: The location where bones come together. (Chapter 13.2)
Jugular Vein Distension (JVD): Occurs when the increased pressure of the superior vena cava causes the jugular vein to bulge, making it most visible on the right side of a person’s neck. (Chapter 9.3)

Keloid: A raised scar caused by overproduction of scar tissue. (Chapter 14.3)
Ketoacidosis: A life-threatening complication of hyperglycemia that can occur in patients with type 1 diabetes mellitus that is associated with symptoms such as fruity-smelling breath, nausea, vomiting, severe thirst, and shortness of breath. (Chapter 19.2)
Key part: Any sterile part of equipment used during an aseptic procedure, such as needle hubs, syringe tips, dressings, etc. (Chapter 4.3)
Key site: The site contacted during an aseptic procedure, such as nonintact skin, a potential insertion site, or an access site used for medical devices connected to the patients. Examples of key sites include the insertion or access site for intravenous (IV) devices, urinary catheters, and open wounds. (Chapter 4.3)
Kinesthesia: A person’s sense of movement. (Chapter 6.2)
Korotkoff sounds: The audible sounds of blood pressure named after Dr. Korotkoff who discovered them. (Chapter 3.2)
Kyphosis: Outward curvature of the back; often described as “hunchback.” (Chapter 10.3, Chapter 13.3)

Lacrimal duct: Tears produced by the lacrimal gland flow through this duct to the medial corner of the eye. (Chapter 8.2)
Laryngopharynx: The portion of the pharynx inferior to the oropharynx and posterior to the larynx that is a passageway for ingested material and air until its inferior end where the digestive and respiratory systems diverge into the esophagus and the larynx. (Chapter 7.2)
Larynx: The structure connecting the pharynx to the trachea that helps regulate the volume of air that enters and leaves the lungs and contains the vocal cords. (Chapter 7.2)
Lens: An inner part of the eye that helps the eye focus. (Chapter 8.2)
Lesion: An area of abnormal tissue. (Chapter 14.2)
Level of consciousness: A patient’s level of arousal and alertness, commonly assessed by asking them to report their name, current location, and time. (Chapter 6.4)
LGBTQ: An acronym standing for lesbian, gay, bisexual, transgender, and queer is an umbrella term that generally refers to a group of people who are diverse with regard to their gender identity and sexual orientation. There are expanded versions of this acronym. (Chapter 2.8)
Ligaments: Strong bands of fibrous connective tissue that connect bones and strengthen and support joints by anchoring bones together and preventing their separation. (Chapter 13.2)
Lordosis: An inward curve of the lumbar spine just above the buttocks. A small degree of lordosis is normal, but too much curving is called swayback. (Chapter 13.3)
Lymphadenopathy: Enlarged lymph nodes. (Chapter 7.3)
Lymphedema: A type of swelling that occurs when lymph fluid builds up in the body’s soft tissues due to damage to the lymph system. (Chapter 14.3)
Lymph nodes: Structures in the lymphatic system that filter pathogens. (Chapter 7.2)

Maceration: The softening and wasting away of skin due to excess fluid. (Chapter 20.2)
Macrodrip tubing: Gravity IV tubing with drop factors of 10, 15, or 20 drops per milliliter that are typically used to deliver general IV solutions for adults. (Chapter 5.14)
Main health care needs: Term used to classify what needs the patient feels are most important to address after admission to a health care agency. (Chapter 2.5)
Mandible: Lower jawbone. (Chapter 7.2)
MAR: Medication administration record contained in a patient’s chart. (Chapter 15.2)
Masseter: Main muscle used for chewing because it elevates the mandible to close the mouth. (Chapter 7.2)
Maturation phase: The final phase of wound healing as collagen continues to be created to strengthen the wound, causing scar tissue. (Chapter 20.2)
Maxilla: Bone that forms the upper jaw and supports the upper teeth. (Chapter 7.2)
Medical asepsis: Measures to prevent the spread of infection in health care agencies. (Chapter 1.2)
Medication cup: A small plastic or paper cup used to dispense oral medications. Some plastic medication cups have calibration marks for measuring medication amounts. (Chapter 5.2)
Medication reconciliation: A comparison of a list of current medications with a previous list and is completed at every hospitalization and clinic visit. (Chapter 2.6)
Melanin: Skin pigment produced by melanocytes scattered throughout the epidermis. (Chapter 14.2)
Melanoma: Skin cancer characterized by the uncontrolled growth of melanocytes that commonly develops from a mole. Melanoma is the most fatal of all skin cancers because it is highly metastatic. Melanomas usually appear as asymmetrical brown and black patches with uneven borders and a raised surface. (Chapter 14.3)
Melena: Dark, tarry-looking stool due to the presence of digested blood. (Chapter 2.8, Chapter 12.3)
Mental health: A state of well-being in which every individual realizes their own potential, can cope with normal stresses of life, can work productively and fruitfully, and is able to make a contribution to their community. (Chapter 2.8)
Metered dose inhaler (MDI): An inhaler that provides a mist of medication that is inhaled through the mouth into the lungs. An example of an MDI is albuterol. Optimal administration is achieved with a spacer attached to the inhaler. (Chapter 16.2)
Microdrip tubing: Gravity IV tubing with a drop factor of 60 drops per milliliter. (Chapter 5.14)
Midline peripheral catheters: A larger catheter (i.e., 16-18 gauge) that allow for rapid infusions but does not terminate in the central vasculature. (Chapter 23.2)
Military time: A method of measuring the time based on the full 24 hours of the day rather than two groups of 12 hours indicated by AM and PM. (Chapter 5.3)
Mobility: A patient’s ability to move around (e.g., sit up, sit down, stand up, walk). (Chapter 2.8)
Motor nerves: Nerves in the peripheral nervous system that transmit motor signals from the brain to the muscles to cause movement. (Chapter 6.2)
Murmur: A blowing or whooshing sound heard on auscultation of the precordium that signifies turbulent blood flow in the heart often caused by a valvular defect. (Chapter 9.3)
Muscle atrophy: The thinning or loss of muscle tissue that can be caused by disuse, aging, or neurological damage. (Chapter 13.2)
Myopia: Impaired vision, also known as nearsightedness, that makes far-away objects look blurry. (Chapter 8.2)

Nares: Nostril openings into the nasal cavity. (Chapter 7.2)
Nasal septum: Bone and cartilage that separate the nasal cavity into two compartments. (Chapter 7.2)
Nasopharynx: The upper region of the pharynx that connects to the nasal cavity and is a passageway for air. (Chapter 7.2)
Necrosis: Tissue death. (Chapter 23.2)
Necrotic: Black tissue color due to tissue death from lack of oxygenation to the area. (Chapter 20.2)
Nonblanchable erythema: Skin redness that does not turn white when pressure is applied. (Chapter 20.2)

One-time order: A prescription for a medication to be administered only once. An example of a one-time order is a prescription for an IV dose of antibiotics to be administered immediately prior to surgery. (Chapter 15.2)
Open fracture: A type of fracture when the broken bone punctures the skin. (Chapter 13.3)
Optic nerve: Cranial nerve II that conducts visual information from the retina to the brain. (Chapter 8.2)
Oral syringe: A specific type of syringe used to measure and/or administer medications via the oral route. (Chapter 5.2)
Orbit: The bony socket that houses the eyeball and muscles that move the eyeball. (Chapter 7.2)
Oropharyngeal suctioning: Suction of secretions through the mouth, often using a Yankauer device. (Chapter 22.4)
Oropharynx: The part of the throat at the back of the mouth behind the oral cavity. It includes the back third of the tongue, the soft palate, the side and back walls of the throat, and the tonsils. (Chapter 7.2, Chapter 19.4)
Orthopnea: Breathlessness or a feeling of shortness of breath when lying in a reclined position. (Chapter 9.3, Chapter 10.3)
Orthostatic hypotension: A decrease in blood pressure by at least 20 mmHg systolic or 10 mmHg diastolic within three minutes of standing from a seated or lying position. (Chapter 3.2)
Osteoarthritis: The most common type of arthritis associated with aging and wear and tear of the articular cartilage that covers the surfaces of bones at the synovial joint. (Chapter 13.3)
Osteomyelitis: Bone infection. (Chapter 20.2)
Osteoporosis: A disease that thins and weakens bones, especially in the hip, spine, and wrist, causing them to become fragile and break easily. (Chapter 13.3)
Ostomy: The surgical procedure that creates the opening from the stoma outside the body to an organ such as the small intestine, colon, rectum, or bladder. A stoma can be permanent, such as when an organ is removed, or temporary, such as when an organ requires time to heal. (Chapter 21.7)
Otitis externa: The medical diagnosis for external ear inflammation and/or infection. (Chapter 8.2)
Ototoxic medications: Medications that cause the adverse effect of sensorineural hearing loss by affecting the hair cells in the cochlea. (Chapter 8.2)
Outer cannula: The outer cannula placed by the provider through the tracheostomy stoma and continuously remains in place. (Chapter 22.2)
P
Pallor: A reduced amount of oxyhemoglobin in the skin or mucous membranes. Skin and mucous membranes present with a pale skin color. (Chapter 9.3, Chapter 10.3, Chapter 14.4)
PaO2: The partial pressure of dissolved oxygen in the blood measured by arterial blood gas samples. (Chapter 11.2)
Paralysis: The partial or complete loss of strength, movement, or control of a muscle or group of muscles within a body part that can be caused by brain or spinal injury. (Chapter 6.5)
Paroxysmal nocturnal dyspnea: An attack of severe shortness of breath that generally occurs at night. (Chapter 9.3)
Passive range of motion: The degree of range of motion a patient demonstrates in a joint when the examiner is providing the movement. (Chapter 13.4)
Percussion: An advanced physical examination technique where body parts are tapped with fingers to determine their size and if fluid is present. (Chapter 1.1)
Perfusion: The amount of blood flow to tissue. (Chapter 9.3)
Pericardial friction rub: Uncommon heart sounds produced when the parietal and visceral pericardium become inflamed, generating a creaky-scratchy noise as they rub together. (Chapter 9.3)
Peripheral edema: Swelling due to an accumulation of fluid in tissues perfused by the peripheral vascular system. (Chapter 9.3)
Peripheral inserted central catheter: A thin, flexible tube inserted into a vein in the upper arm and guided into the superior vena cava. (Chapter 23.2)
Peripheral IV: An intravenous catheter inserted by percutaneous venipuncture into a peripheral vein and held in place with a sterile transparent dressing. (Chapter 23.2)
Peripheral nervous system: The part of the nervous system that includes the cranial and spinal nerves. (Chapter 6.2)
Peripheral neuropathy: A condition that causes decreased sensation of pain and pressure, typically in the lower extremities. (Chapter 20.2)
Periwound: The skin around the outer edges of a wound. (Chapter 20.3)
Personal Protective Equipment (PPE): Includes gloves, gowns, goggles, face shields, and masks, along with environmental controls, to prevent the transmission of infection for patients who are diagnosed or suspected of having an infectious disease. (Chapter 1.2, Chapter 4.1)
Petechiae: Tiny red dots caused by bleeding under the skin. (Chapter 14.3)
Pharyngitis: Infection and/or inflammation in the back of the throat (pharynx). (Chapter 7.3)
Pharynx: A tube lined with mucous membrane that begins at the nasal cavity and is divided into three major regions: the nasopharynx, the oropharynx, and the laryngopharynx. (Chapter 7.2)
Phlebitis: Inflammation of a vein. (Chapter 23.4)
Physical examination: A systematic data collection method of the body that uses the techniques of inspection, auscultation, palpation, and percussion. (Chapter 1.1)
Pitting edema: An accumulation of fluid in tissue and causes an indentation when the area is pressed. (Chapter 9.3)
Pleural rub: Uncommon sound heard when there is inflammation of the lung pleura, resulting in friction as the surfaces rub against each other. (Chapter 10.3)
Precordium: The region of the thorax in front of the heart. (Chapter 9.3)
Presbycusis: Sensorineural hearing loss that occurs with aging due to gradual nerve degeneration. (Chapter 8.2)
Presbyopia: Impaired near vision that commonly occurs in middle-aged and older adults. (Chapter 8.2)
Prescriptions: Orders, interventions, remedies, or treatments ordered or directed by an authorized primary health care provider. (Chapter 15.2)
Pressure injury: Skin breakdown caused when a patient’s skin and soft tissue press against a hard surface for a prolonged period of time, causing reduced blood supply and resulting in damaged tissue. (Chapter 14.3, Chapter 20.2)
Primary intention: Wound healing that occurs with surgical incisions or clean-edged lacerations that are closed with sutures, staples, or surgical glue. (Chapter 20.2)
Primary source of data: Information obtained directly from the patient. (Chapter 1.1, Chapter 2.2)
Primary survey: A brief observation at the start of a shift or visit to verify the patient is stable by assessing mental status, airway, breathing, and circulation. (Chapter 1.4)
PRN order: A prescription for medication to be administered when it is requested by, or as needed by, the patient. PRN orders are usually administered based on patient symptoms such as pain medications. An example of a PRN order is a prescription for pain medication, such as “Acetaminophen 500 mg PO every 4-6 hours as needed for pain.” (Chapter 15.2)
Proliferative phase: The third phase of wound healing that includes epithelialization, angiogenesis, collagen formation, and contraction. (Chapter 20.2)
Proprioception: A person’s sense of their body position. (Chapter 6.2, Chapter 6.8)
Prostate hypertrophy: A common medical condition of the enlargement of the prostate gland in males as they age, causing uncomfortable urinary symptoms such as urgency and frequency. (Chapter 12.3)
Protuberant: Convex or bulging appearance. (Chapter 12.3)
Ptosis: Drooping of the eyelid. (Chapter 6.5)
Pulmonary embolism: A blood clot that lodges in one of the arteries that go from the heart to the lung. (Chapter 9.3)
Pupil: The hole at the center of the eye that allows light to enter. (Chapter 8.2)
Purulent drainage: Wound exudate that is thick and opaque and can be tan, yellow, green, or brown in color. It is never considered normal in a wound, and new purulent drainage should always be reported to the health care provider. (Chapter 20.3)
R
Rales: Another term used for crackles. (Chapter 10.3)
Rebound tenderness: Pain when hand is withdrawn during palpation. (Chapter 12.3)
Reconstitution: The process of adding a liquid diluent to a dry ingredient to make a liquid in a specific concentration. (Chapter 5.10)
Respiration: Includes ventilation and gas exchange at the alveolar level where blood is oxygenated and carbon dioxide is removed. (Chapter 10.2)
Retina: The nervous tissue and photoreceptors in the eye that initially process visual stimuli. (Chapter 8.2)
Retractions: The “pulling in” of muscles between the ribs or in the neck when breathing, indicating difficulty breathing or respiratory distress. (Chapter 10.3)
Rheumatoid arthritis: A type of arthritis that causes pain, swelling, stiffness, and loss of function in joints due to inflammation caused by an autoimmune disease. (Chapter 13.3)
RICE: A mnemonic for treatment of sprains and strains that stands for: Resting the injured area, Icing the area, Compressing the area with an ACE bandage or other device, and Elevating the affected limb. (Chapter 13.3)
Rigidity: Involuntary contraction of the abdominal musculature in response to peritoneal inflammation. (Chapter 12.3)
Rotation: Circular movement of a joint around a fixed point. (Chapter 13.2)
Routine order: A written prescription that is followed until another order cancels it. An example of a routine order is a prescription for daily medication such as “Lisinopril 10 mg PO daily.” (Chapter 15.2)
Rule of Nines: A tool used in the emergency department to assess the total body surface area burned to quickly estimate intravenous fluid requirements. (Chapter 14.3)

Safety culture: A culture established in health care agencies to empower staff to speak up about risks to patients and to report errors and near misses, all of which drive improvement in patient care and reduce the incident of patient harm. (Chapter 15.2)
Saline lock: Refers to the use of a short extension set that allows IV access without requiring ongoing IV infusions. (Chapter 23.2)
Sanguineous drainage: Wound drainage that is fresh bleeding. (Chapter 20.3)
SaO2: Calculated arterial oxygen saturation level. (Chapter 11.2)
Scaphoid: Sunken appearance. (Chapter 12.3)
Sclera: White area of the eye. (Chapter 8.2)
Scoliosis: A sideways curve of the spine that commonly develops in late childhood and the early teens. (Chapter 13.3)
Secondary intention: Wound healing that occurs when the edges of a wound cannot be approximated (brought together), so the wound fills in from the bottom up by the production of granulation tissue. Second-degree burn: Burn affecting both the epidermis and a portion of the dermis, resulting in swelling and a painful blistering of the skin. (Chapter 14.3)
Self-concept: Knowledge a person has about themselves that makes up who they are (i.e., their identity). (Chapter 2.8)
Self-esteem: A person’s self-evaluation of their self-concept as being worthy or unworthy. (Chapter 2.8)
Sensation: The function of receiving information about the environment. The major senses are taste, smell, touch, sight, and hearing. (Chapter 6.2)
Sensorineural hearing loss: Hearing loss caused by pathology of the inner ear, cranial nerve VIII, or auditory areas of the cerebral cortex. (Chapter 8.2)
Sensory nerves: Nerves in the peripheral nervous system that carry impulses from the body to the brain for processing. (Chapter 6.2)
Serosanguinous drainage: Wound exudate contains serous drainage with small amounts of blood present. (Chapter 20.3)
Serous drainage: Wound drainage that is clear, thin, watery plasma. It is considered normal in minimal amounts during the inflammatory stage of wound healing. (Chapter 20.3)
Sexual orientation: The preferred term used when referring to an individual’s physical and/or emotional attraction to the same and/or opposite gender. (Chapter 2.8)
Shear: A mechanical force that occurs when tissue layers move over the top of each other, causing blood vessels to stretch and break as they pass through the subcutaneous tissue. (Chapter 20.2)
Sign: Objective data found by the nurse or health care provider when assessing a patient. (Chapter 2.2)
Sinusitis: Inflamed sinuses caused by a viral or bacterial infection. (Chapter 7.3)
Skeletal muscle: Voluntary muscle that produces movement, assists in maintaining posture, protects internal organs, and generates body heat. (Chapter 13.2)
Skin tears: Wounds caused by mechanical forces, typically in the nonelastic skin of older adults. (Chapter 20.2)
Skin turgor: The skin’s elasticity and its ability to change shape and return to normal when gently grasped between two fingers. (Chapter 14.4)
Slough: Inflammatory exudate that is light yellow, soft, and moist and must be removed for wound healing to occur. (Chapter 20.2)
Small-volume nebulizers: Devices that provide a fine mist using oxygen or compressed air to transport the medication from a nebulizer cup into the mouth and into the lungs as the patient breathes normally through a mask or pipe device. (Chapter 16.2)
Snellen chart: A chart used to test far vision. (Chapter 8.2)
Sphygmomanometer: A device used to measure blood pressure and is commonly referred to as a blood pressure cuff. (Chapter 3.2, Chapter 9.3)
Spirituality: A way of living that comes from a set of meanings, values, and beliefs that are important to a person. (Chapter 2.8)
SpO2: An estimated oxygenation level based on the saturation level of hemoglobin measured by a pulse oximeter. (Chapter 11.2)
Sprain: A stretched or torn ligament caused by an injury. (Chapter 13.3)
Standardized sliding-scale insulin protocol: Standardized instructions for administration of adjustable insulin dosages based on a patient’s premeal blood glucose readings. (Chapter 19.2)
Standard precautions: The minimum infection prevention practices that apply to all patient care, regardless of suspected or confirmed infection status of the patient, in any setting where health care is delivered. (Chapter 4.2)
Standing order: Standing orders are standard prescriptions for nurses to implement for patients in clearly defined circumstances without the need to notify a provider. They may also be referred to as an “order set” or a “protocol.” An example of a standing order/protocol is a standard prescription for all patients coming into an urgent care reporting chest pain to immediately receive four chewable aspirin, the placement of an IV, and an electrocardiogram (ECG). (Chapter 15.2)
STAT order: A one-time prescription that is administered without delay. An example of a STAT order is the prescription for a dose of Benadryl to be administered to a patient having an allergic reaction. (Chapter 15.2)
Stereognosis: The ability to perceive the physical form and identity of an object based on tactile stimuli alone. (Chapter 6.6)
Sterile technique (surgical asepsis): Techniques used to eliminate every potential microorganism in and around a sterile field while maintaining objects and areas as free from microorganisms as possible. This technique is the standard of care for surgical procedures, invasive wound management, and central line care. (Chapter 4.3)
Sternocleidomastoid: The major muscle that laterally flexes and rotates the head. (Chapter 7.2)
Stoma: An opening on the abdomen that is connected to the gastrointestinal or urinary systems to allow waste (urine or feces) to be collected in a pouch. (Chapter 21.7)
Stomach decompression: Using suctioning through a nasogastric tube to remove the contents of the stomach. (Chapter 17.1)
Straight catheter: A catheter used for intermittent urinary catheterization; it does not have a balloon at the insertion end. (Chapter 21.2)
Strain: A stretched or torn muscle or tendon. (Chapter 13.3)
Striae: White or silver markings from stretching of the skin. (Chapter 12.3)
Stridor: High-pitched crowing sounds heard over the upper airway and larynx indicating obstruction. (Chapter 10.3)
Subcutaneous injection: Medication administered into the subcutaneous tissue just under the dermis. (Chapter 18.1)
Subjective data: Information obtained from the patient and/or family members that offers important cues from their perspectives. (Chapter 1.1, Chapter 2.2)
Suction canister: A container for collecting suctioned secretions that is attached to a suction source. (Chapter 22.4)
Suction catheter: A soft, flexible, sterile catheter used for nasopharyngeal and tracheostomy suctioning. (Chapter 22.4)
Suture: An interlocking joint between adjacent bones of the skull. (Chapter 7.2)
Symptom: Subjective data that the patient reports, such as “I feel dizzy.” (Chapter 2.2)
Syncope: A temporary loss of consciousness usually related to insufficient blood flow to the brain. (Chapter 9.3)
Synovial fluid: A thick fluid that provides lubrication in joints to reduce friction between the bones. (Chapter 13.2)
Synovial joints: A fluid-filled joint cavity where the articulating surfaces of the bones contact and move smoothly against each other. The elbow and knee are examples of synovial joints. (Chapter 13.2)
Syringe: A medical device used to administer parenteral medication into tissue or into the bloodstream. Syringes can also be used to withdraw blood or fluid. (Chapter 5.2)
Systole: The phase of the heartbeat when the left ventricle contracts and pumps blood into the arteries. (Chapter 3.2)
Systolic blood pressure: The maximum pressure of blood on the arteries during the contraction of the left ventricle of the heart referred to as systole. (Chapter 3.2)
T
Tachypnea: Rapid and often shallow breathing greater than normal range according to the patient’s age. (Chapter 10.3)
Temporalis: Muscle that assists in chewing by retracting the mandible. The temporalis muscle can be felt moving by placing fingers on the patient’s temple as they chew. (Chapter 7.2)
Tendons: Strong bands of dense, regular connective tissue that connect muscles to bones. (Chapter 13.2)
Tertiary intention: Wound healing that occurs when a wound must remain open or has been reopened, often due to severe infection. (Chapter 20.2)
Thalamus: Relays sensory information and motor information in collaboration with the cerebellum. (Chapter 6.2)
Third-degree burn: Severe burn that fully extends into the epidermis and dermis, destroying the tissue and affecting the nerve endings and sensory function. (Chapter 14.3)
Thrill: A vibration felt with palpation of the precordium. (Chapter 9.3)
Tinnitus: Ringing, buzzing, roaring, hissing, or whistling sound in the ears. (Chapter 8.2)
Titration order: An order in which the medication dose is either progressively increased or decreased by the nurse in response to the patient’s status. (Chapter 15.2)
Topical medications: Medications administered via the skin or mucous membranes for direct local action, as well as for systemic effects. (Chapter 16.2)
Total parenteral nutrition (TPN): A very concentrated solution that must be administered via a central line. (Chapter 23.2)
Trachea: A tube lined with mucus membrane that carries air from the larynx to the lungs. (Chapter 7.2)
Tracheostomy: A surgically created opening that goes from the front of the neck into the trachea. (Chapter 22.2)
Tracheostomy dressing: A manufactured dressing used with tracheostomies that does not shed fibers, which could potentially be inhaled by the patient. (Chapter 22.4)
Transdermal route: Patches or disks applied to the skin that deliver medication over an extended period of time. (Chapter 16.2)
Trapezius: The muscle that elevates the shoulders (shrugs), pulls the shoulder blades together, and tilts the head backwards. (Chapter 7.2)
Tunneling: Passageways underneath the surface of the skin that extend from a wound and can take twists and turns. (Chapter 20.2)
Tympanic membrane: The membrane at the end of the external ear canal, commonly called the eardrum, that vibrates after it is struck by sound waves. (Chapter 8.2)
U
Undermining: A condition that occurs in wounds when the tissue under the wound edges becomes eroded, resulting in a pocket beneath the skin at the wound’s edge. (Chapter 20.2)
Unstageable: Occurs when slough or eschar obscures the wound so that tissue loss cannot be assessed. (Chapter 20.2)
Urinary catheterization: The insertion of a catheter tube into the urethral opening and placing it in the neck of the urinary bladder to drain urine. (Chapter 21.2)
Urinary frequency: Urination every hour or two. (Chapter 12.3)
Urinary incontinence: Involuntary leakage of urine. (Chapter 12.3)
Urinary tract infection (UTI): An infection in the urinary system causing symptoms such as burning on urination (dysuria), frequent urination, malodorous urine, fever, and change in level of consciousness. (Chapter 21.2)
Urinary urgency: An intense urge to urinate that can lead to urinary incontinence. (Chapter 12.3)
Urostomy: The ureters (tubes that carry urine from the kidney to the bladder) are attached to a stoma to bypass the bladder. (Chapter 21.7)
Uvula: A small, teardrop-shaped structure located at the apex of the soft palate that swings upward during swallowing to close off the nasopharynx and prevent ingested materials from entering the nasal cavity. (Chapter 7.2)
V
Value: An accepted principle or standard of an individual or group. (Chapter 2.8)
Vastus lateralis: A muscle located on the anterior lateral aspect of the thigh and extends from one hand’s breadth above the knee to one hand’s breadth below the greater trochanter. It is commonly used for immunizations in infants and toddlers because the muscle is thick and well-developed. (Chapter 18.6)
Venipuncture: The process of introducing a needle into a client’s vein to collect a blood sample or insert an IV catheter. (Chapter 23.2)
Venous insufficiency: A medical condition where the veins in the legs do not adequately send blood back to the heart, resulting in a pooling of fluids in the legs that can cause venous ulcers. (Chapter 20.2)
Venous ulcers: Ulcers caused by the pooling of fluid in the veins of the lower legs when the valves are not working properly, causing fluid to seep out, macerate the skin, and cause an ulcer. (Chapter 20.2)
Ventilation: The mechanical movement of air into and out of the lungs. (Chapter 10.2, Chapter 11.3)
Ventrogluteal: The safest intramuscular injection site for adults and children because it provides the greatest thickness of gluteal muscles, is free from penetrating nerves and blood vessels, and has a thin layer of fat. (Chapter 18.6)
Vertigo: A type of dizziness often described by patients as “the room feels as if it is spinning.” (Chapter 8.2)
Vesicular sounds: Low-pitched soft sounds like “rustling leaves” heard over alveoli and small bronchial airways. (Chapter 10.3)
Vestibulocochlear nerve: Cranial nerve VIII that transports neural signals from the cochlea and the vestibule to the brain stem regarding hearing and balance. (Chapter 8.2)
Voiding: Medical terminology used for urinating. (Chapter 2.8)
Voluntary guarding: Voluntary contraction of abdominal wall musculature; may be related to fear, anxiety, or presence of cold hands. (Chapter 12.3)

Wheeze: High-pitched sounds heard on expiration or inspiration associated with bronchoconstriction or bronchospasm. (Chapter 10.3)
Wound vac: A device used with special foam dressings and suctioning to remove fluid and decrease air pressure around a wound to assist in healing. (Chapter 20.5)
Y
Yankauer suction tip: Rigid device used to suction secretions from the mouth. (Chapter 22.4)
Z
Z-track method: A method for administering intramuscular injections that prevents the medication from leaking into the subcutaneous tissue, allows the medication to stay in the muscles, and minimizes irritation. (Chapter 18.6)

Abduction: Joint movement away from the midline of the body. (Chapter 13.2)
Accessory muscles: Muscles other than the diaphragm and intercostal muscles that may be used for labored breathing. (Chapter 10.3)
Accommodation: The ability of the eye to adjust from near vision to far vision. Pupils constrict at near vision and dilate at far vision. (Chapter 6.5)
Acne: A skin disturbance that typically occurs on areas of the skin that are rich in sebaceous glands, such as the face and back. (Chapter 14.3)
Active range of motion: The degree of movement a patient can voluntarily achieve in a joint without assistance. (Chapter 13.4)
Activities of daily living: Daily basic tasks fundamental to everyday functioning (e.g., hygiene, elimination, dressing, eating, ambulating/moving). (Chapter 2.8)
Acute otitis media: The medical diagnosis for a middle ear infection. (Chapter 8.2)
Adduction: Joint movement toward the middle line of the body. (Chapter 13.2)
Affect: Outward display of one’s emotional state. A “flat” affect with little display of emotion is associated with depression. (Chapter 1.4)
AIDET: Mnemonic for introducing oneself in health care that includes Acknowledge, Introduce, Duration, Explanation, and Thank You. (Chapter 1.2)
Airborne precautions: Infection prevention and control interventions to be used in addition to standard precautions for diseases spread by airborne transmission, such as measles and tuberculosis. (Chapter 4.2)
Ampules: Small glass containers of liquid medication ranging from 1 mL to 10 mL sizes. (Chapter 18.2)
Angiogenesis: The development of new capillaries in a wound base. (Chapter 20.2)
Anosmia: Partial or complete loss of smell. This symptom can be related to underlying cranial nerve dysfunction or other nonpathological causes such as a common cold. (Chapter 6.5)
Apnea: Absence of respirations. (Chapter 10.3)
Apocrine sweat gland: Sweat glands associated with hair follicles in densely hairy areas that release organic compounds subject to bacterial decomposition causing odor. (Chapter 14.2)
Around the Clock (ATC) order: An order that reflects that medication should be administered at regular time intervals, such as every six hours, to maintain consistent levels of the drug in the patient’s bloodstream. (Chapter 15.2)
Arterial blood gas (ABG): A sample of arterial blood that measures the oxygen, carbon dioxide, and bicarbonate levels. (Chapter 11.2)
Arterial blood sampling: Blood is obtained via puncture into an artery by specially trained registered nurses and other health care personnel, such as respiratory therapists, physicians, nurse practitioners, and physician assistants. (Chapter 23.2)
Arterial ulcers: Ulcers caused by lack of blood flow and oxygenation to tissues and typically occur in the distal areas of the body such as the feet, heels, and toes. (Chapter 20.2)
Arthroplasty: Joint replacement surgery. (Chapter 13.3)
Arthroscopic surgery: A surgical procedure involving a small incision and the insertion of an arthroscope, a pencil-thin instrument that allows for visualization of the joint interior. Small surgical instruments are inserted via additional incisions to remove or repair ligaments and other joint structures. (Chapter 13.3)
Articular cartilage: Smooth, white tissue that covers the ends of bones where they come together at joints, allowing them to glide over each other with very little friction. Articular cartilage can be damaged by injury or normal wear and tear. (Chapter 13.2)
Asepsis: A state of being free of disease-causing microorganisms. (Chapter 4.3)
Aseptic non-touch technique: A standardized technique, supported by evidence, to maintain asepsis and standardize practice. (Chapter 4.3)
Aseptic technique (medical asepsis): The purposeful reduction of pathogen numbers while preventing microorganism transfer from one person or object to another. This technique is commonly used to perform invasive procedures, such as IV starts or urinary catheterization. (Chapter 4.3)
Aspiration pneumonia: A type of lung infection caused by material from the stomach or mouth inadvertently entering the lungs that can be life-threatening. (Chapter 7.3, Chapter 12.3)
Atelectasis: Alveoli or an entire lung is collapsed, allowing no air movement. (Chapter 10.3)
Atrial fibrillation: An irregular heartbeat that is often fast and increases the risk of heart attack or stroke. (Chapter 9.3)
Auricle: The large, fleshy structure of the ear on the lateral aspect of the head. (Chapter 8.2)
Auscultation: Listening to sounds, such as heart, lung, and bowel sounds, created by organs using a stethoscope. (Chapter 1.1)
B
Babinski response: A reflex demonstrated as fanning of 4 toes with great toe bending towards top of foot when object slid along sole of foot. Normal in children under 2. Older than 2 through adulthood, all five toes should curl downward. (Chapter 6.9)
Barrel-chested: An equal AP-to-transverse diameter that often occurs in patients with COPD due to hyperinflation of the lungs. (Chapter 10.3)
Belief: Something accepted as true with a sense of certainty. (Chapter 2.8)
Bladder scan: A bedside test using a noninvasive tool used to measure the volume of urine in the bladder. (Chapter 21.6)
Blanching: To make white or pale by applying pressure. (Chapter 9.3, Chapter 14.4)
Bleb: A small, raised circle that appears after administration of an intradermal medication indicating correct placement into the dermis. (Chapter 18.4)
Body image: A mental picture of one’s body related to appearance and function. (Chapter 2.8)
Body Mass Index (BMI): A standardized reference range to gauge a patient’s weight status. (Chapter 1.4)
Borborygmus: Hyperperistalsis, often referred to as “stomach growling.” (Chapter 12.3)
Bradypnea: Decreased respiratory rate or slow breath less than normal range according to the patient’s age. (Chapter 10.3)
Broca’s area: An area located in the frontal lobe that is responsible for the production of language and controlling movements responsible for speech. (Chapter 6.2)
Bronchial breath sounds: High-pitched hollow sounds heard over trachea and the larynx. (Chapter 10.3)
Bronchovesicular sounds: Mixture of low- and high-pitched sounds heard over major bronchi. (Chapter 10.3)
Bruit: A swishing sound heard upon auscultation. (Chapter 6.10, Chapter 9.3)
C
Candidiasis: A fungal infection often referred to as “thrush” when it occurs in the oral cavity in children. (Chapter 7.3)
Capillary blood testing: Blood collected from capillaries located near the surface of the skin. (Chapter 23.2)
Capillary refill: The time it takes for color to return after pressure is applied to tissue causing blanching. (Chapter 9.3)
Care partners: Family and friends who are involved in helping to care for the patient. (Chapter 2.2)
Catheter embolism: Occurs when a small part of the cannula breaks off and flows into the vascular system. (Chapter 23.4)
CAUTI: Catheter-associated urinary tract infection. (Chapter 21.3)
Central nervous system: The part of the nervous system that includes the brain (the interpretation center) and the spinal cord (the transmission pathway). (Chapter 6.2)
Central venous access device (CAVD): A type of vascular access that involves the insertion of a catheter into a large vein in the arm, neck, chest, or groin. (Chapter 23.2)
Cerebellum: The part of the brain that coordinates skeletal and smooth muscle movement and maintains equilibrium and balance. (Chapter 6.2)
Cerebral cortex: The cerebrum is covered by a wrinkled outer layer of gray matter. (Chapter 6.2)
Cerumen impaction: A buildup of earwax causing occlusion of the ear canal. (Chapter 8.2)
Chief complaint: The reason a patient is seeking health care during a visit to a clinic or on admission to a health care facility. (Chapter 2.5)
Cleft lip: A birth defect caused by a partial or complete failure of the right and left portions of the upper lip to fuse together, leaving a gap in the lip. (Chapter 7.3)
Cleft palate: A birth defect caused when two halves of the hard palate fail to completely come together and fuse at the midline, leaving a gap between them, and making it very difficult for an infant to generate the suckling needed for nursing. (Chapter 7.3)
Click: Clicking sound heard on auscultation of the precordium; often heard in patients with heart valve abnormalities. (Chapter 9.3)
Clubbing: A change in the configuration where the tips of the nails curve around the fingertips, usually caused by chronic low levels of oxygen in the blood. (Chapter 10.3, Chapter 11.2)
Clubfoot: A congenital condition that causes the foot and lower leg to turn inward and downward. (Chapter 13.3)
Colostomy: The colon is attached to a stoma to bypass the rectum and the anus. (Chapter 21.7)
Comatose: A decreased level of consciousness with a score of less than 8 on the Glasgow Coma Scale. (Chapter 6.4)
Concussion: A type of traumatic brain injury caused by a bump, blow, or jolt to the head or by a hit to the body that causes the head and brain to move rapidly back and forth. This sudden movement can cause the brain to bounce around or twist in the skull, creating chemical changes in the brain and damaging brain cells. (Chapter 7.3)
Conductive hearing loss: Hearing loss that occurs when something in the external or middle ear is obstructing the transmission of sound. (Chapter 8.2)
Congenital condition: A condition present at birth. (Chapter 13.3)
Conjunctiva: Inner surface of the eyelid. (Chapter 8.2)
Conjunctivitis: A viral or bacterial infection in the eye causing swelling and redness in the conjunctiva and sclera. (Chapter 8.2)
Contact precautions: Infection prevention and control interventions to be used, in addition to standard precautions, for diseases spread by contact with the patient, their body fluids, or their surroundings, such as C-diff, MRSA, VRE, and RSV. (Chapter 4.2)
Contracture: A fixed or permanent tightening of muscles, tendons, ligaments, or the skin that prevents normal movement of the body part. (Chapter 13.3)
Convergence: The action of both eyes moving inward as they focus on a close object using near vision. (Chapter 6.5)
Cornea: The transparent front part of the eye that covers the iris, pupil, and anterior chamber. (Chapter 8.2)
Coude catheter: A catheter specifically designed to maneuver around obstructions or blockages in the urethra such as with enlarged prostate glands in males. Coude originates from the French word that means “bend.” (Chapter 21.2)
Crackles: Also referred to as “rales”; sound like popping or crackling noises during inspiration. Associated with inflammation and fluid accumulation in the alveoli. (Chapter 10.3)
Cranium: Eight bones that protect the brain in the cranial cavity. (Chapter 7.2)
Crepitus: A crackling, popping noise heard on joint movement. It is considered normal when it is not associated with pain. (Chapter 10.3, Chapter 13.2)
Cultural safety: The creation of safe spaces for patients to interact with health professionals without judgment, racial reductionism, racialization, or discrimination. (Chapter 1.2, Chapter 2.2)
Cyanosis: Bluish discoloration of the skin, lips, and nail beds. It is an indication of decreased perfusion and oxygenation. (Chapter 9.3, Chapter 10.3, Chapter 11.2, Chapter 14.4)

Debridement: The removal of nonviable tissue in a wound. (Chapter 20.4)
Deep Vein Thrombosis (DVT): A blood clot that forms in a vein deep in the body. (Chapter 9.3)
Dehiscence: The separation of the edges of a surgical wound. (Chapter 20.2)
Deltoid: Commonly used for intramuscular vaccinations in adults because it has a triangular shape and is easy to locate and access. The injection site is in the middle of the deltoid muscle, about 1 to 2 inches below the acromion process. (Chapter 18.6)
Dermatome: An area of the skin that is supplied by a single spinal nerve. (Chapter 6.6)
Dermis: The inner layer of skin with connective tissue, blood vessels, sweat glands, nerves, hair follicles, and other structures. (Chapter 14.2)
Developmental stages: A person’s life span can be classified into nine categories of development, including Prenatal Development, Infancy and Toddlerhood, Early Childhood, Middle Childhood, Adolescence, Early Adulthood, Middle Adulthood, Late Adulthood, and Death and Dying. (Chapter 1.2)
Diabetic ulcers: Ulcers that typically develop on the plantar aspect of the feet and toes of patients with diabetes due to lack of sensation of pressure or injury. (Chapter 20.2)
Diaphoretic: Excessive, abnormal sweating. (Chapter 14.4)
Diastole: The phase between each contraction of the heart when the ventricles are filling with blood. (Chapter 3.2)
Diastolic blood pressure: The resting pressure of blood on the arteries between each cardiac contraction. (Chapter 3.2)
Dimensional analysis: Dimensional analysis is a problem-solving technique where measurements are converted to a different (but equivalent) unit of measure by multiplying with a fractional form of 1 to obtain a desired unit of administration. (Chapter 5.6)
Diplopia: Double vision (i.e., seeing two images of a single object). (Chapter 6.5)
Dislocation: A joint injury that forces the ends of bones out of position; often caused by a fall or a blow to the joint. (Chapter 13.3)
Distention: An expansion of the abdomen caused by the accumulation of air or fluid. Patients often report “feeling bloated.” (Chapter 12.3)
Doff: To take off or remove personal protective equipment, such as gloves or a gown. (Chapter 4.2)
Don: To put on equipment for personal protection, such as gloves or a gown. (Chapter 4.2)
Drop factor: The number of drops in one mL of solution when fluids or medications are administered using gravity IV tubing. (Chapter 5.14)
Droplet precautions: Infection prevention and control interventions to be used in addition to standard precautions; used for diseases spread by large respiratory droplets such as influenza, COVID-19, or pertussis. (Chapter 4.2)
Dry powder inhaler (DPI): An inhaler with medication provided in a powder form that is inhaled from the mouth into the lungs using a quick breath to activate the medication and move it into the lungs. An example of a DPI is tiotropium (Spiriva). (Chapter 16.2)
Ductus arteriosus: Shunt that connects the pulmonary artery and aorta in the developing fetus. (Chapter 9.3)
Dysphagia: Difficulty swallowing. (Chapter 6.10, Chapter 7.2, Chapter 7.3, Chapter 12.3, Chapter 15.2)
Dyspnea: A feeling of shortness of breath. (Chapter 9.3, Chapter 10.3, Chapter 11.2)
Dysuria: Painful urination. (Chapter 2.8, Chapter 12.3)

Ecchymosis: Bruising that occurs when small veins and capillaries under the skin break. (Chapter 14.4, Chapter 20.2)
Eccrine sweat gland: Sweat gland that produces hypotonic sweat for thermoregulation. (Chapter 14.2)
Edema: Swelling in tissues caused by fluid retention. (Chapter 9.3, Chapter 20.2)
Elimination: Refers to the removal of waste products through the urine and stool. (Chapter 2.8)
eMAR: Electronic medication administration record contained in a patient’s electronic chart. (Chapter 15.2)
Enhanced barrier precautions: An infection control intervention designed to reduce transmission of multidrug-resistant organisms (MDRO’S) in nursing homes. (Chapter 4.2)
Enteral medications: Medications that are administered directly into the gastrointestinal tract orally, rectally, or through a tube such as a nasogastric (NG) tube, nasointestinal (NI) tube, or percutaneous endoscopic gastrostomy (PEG) tube. (Chapter 15.1)
Enteral nutrition (EN): Nutrition provided directly into the gastrointestinal (GI) tract through an enteral tube that bypasses the oral cavity. (Chapter 17.2)
Epidermis: The thin, uppermost layer of skin. (Chapter 14.2)
Epiglottis: A flexible piece of cartilage that covers the opening of the trachea during swallowing to prevent ingested material from entering the trachea. (Chapter 7.2)
Epistaxis: Nosebleed. (Chapter 7.3)
Epithelialization: The development of new epidermis and granulation tissue. (Chapter 20.2)
Equivalency: Two values or quantities that are the same amount. For example, one cup is equivalent to eight ounces. (Chapter 5.4)
Erythema: Redness. (Chapter 14.4, Chapter 20.2, Chapter 20.3)
Eschar: Dark brown/black, dry, thick, and leathery dead tissue in a wound base that must be removed for healing to occur. (Chapter 20.2)
Eustachian tube: The tube connecting the middle ear to the pharynx that helps equilibrate air pressure across the tympanic membrane. (Chapter 8.2)
Extension: Joint movement causing the straightening of limbs (increase in angle) at a joint. (Chapter 13.2)
Extraocular muscles: Six muscles that control the movement of the eye within the orbit. Extraocular muscles are innervated by three cranial nerves, the abducens nerve, the trochlear nerve, and the oculomotor nerve. (Chapter 8.2)
Extravasation: The infiltration of damaging intravenous medications, such as chemotherapy, into the extravascular tissue around the site of infusion, causing tissue injury and possible necrosis. (Chapter 23.4)
Exudate: Fluid that oozes out of a wound; also commonly called pus. (Chapter 20.2)
F
Facial drooping: An asymmetrical facial expression that occurs due to damage of the nerve innervating a particular part of the face. (Chapter 7.2)
Family dynamics: Patterns of interactions between family members that influence family structure, hierarchy, roles, values, and behaviors. (Chapter 1.4)
Fenestrated cannula: Type of tracheostomy tube that contains holes so the patient can speak if the cuff is deflated and the inner cannula is removed. (Chapter 22.2)
15-15 Rule: A rule in an agency’s hypoglycemia protocols that includes providing 15 grams of carbohydrate, and then repeating the blood glucose reading in 15 minutes, and then repeating as needed until the patient’s blood glucose reading is above 70. (Chapter 19.2)
FiO2: Fraction of inspired oxygen (i.e., the concentration of oxygen inhaled). Room air contains 21% oxygen levels, and oxygenation devices can increase the inhaled concentration of oxygen up to 100%. However, FiO2 levels should be decreased as soon as feasible to do so to prevent lung injury. (Chapter 11.2)
First-degree burn: A superficial burn that affects only the epidermis. (Chapter 14.3)
Five moments of hand hygiene: Hand hygiene should be performed during the five moments of patient care: immediately before touching a patient; before performing an aseptic task or handling invasive devices; before moving from a soiled body site to a clean body site on a patient; after touching a patient or their immediate environment; after contact with blood, body fluids, or contaminated surfaces (with or without glove use); and immediately after glove removal. (Chapter 4.2)
Flange: The end of the tracheostomy tube that is placed securely against the patient’s neck. (Chapter 22.2)
Flexion: Joint movement causing the bending of the limbs (reduction of angle) at a joint. (Chapter 13.2)
Fluid volume overload (hypervolemia): A condition when there is too much fluid in the blood. Patients may present with shortness of breath, edema to the extremities, and weight gain. (Chapter 23.2)
Foot drop: The inability to raise the front part of the foot due to weakness or paralysis of the muscles that lift the foot. (Chapter 13.3)
Fourth-degree burn: Severe burn damaging the dermis and the underlying muscle and bone. (Chapter 14.3)
Fracture: A broken bone. (Chapter 13.3)
Functional health: The patient’s physical and mental capacity to participate in activities of daily living (ADLs) and instrumental activities of daily living (IADLs). (Chapter 2.8)
G
Gastric residual volume (GRV): Stomach contents aspirated with a 60-mL syringe, typically performed for patients receiving enteral feeding to assess aspiration risk with associated interventions such as slowing or stopping tube feeding. GRVs in the range of 200–500 mL should raise concern and lead to the implementation of measures to reduce risk of aspiration. (Chapter 17.2)
Gauge: Refers to the diameter of a needle. Gauges can vary from very small diameter (25 to 29 gauge) to large diameter (18 to 22 gauge). (Chapter 18.2)
Gender expression: Characteristics in appearance, personality, and behavior, culturally defined as masculine or feminine. (Chapter 2.8)
Gender identity: One’s basic sense of being male, female, or other gender. (Chapter 2.8)
General survey assessment: A component of a patient assessment that observes the entire patient as a whole. Observation includes using all five senses to gather cues that provide a guideline for additional focused assessments in areas of concern. (Chapter 1.1)
Goiter: An abnormal enlargement of the thyroid gland that can occur with hypothyroidism or hyperthyroidism. (Chapter 7.3)
Gout: A type of arthritis that causes swollen, red, hot, and stiff joints due to the buildup of uric acid, commonly starting in the big toe. (Chapter 13.3)
Granulation tissue: New connective tissue in a wound base with fragile, thin-walled capillaries that must be protected. (Chapter 20.2)
Guarding: Voluntary contraction of abdominal wall musculature; may be related to fear, anxiety, or presence of cold hands. (Chapter 12.3)
H
Hand hygiene: A way of cleaning one’s hands to substantially reduce the number of pathogens and other contaminants (e.g., dirt, body fluids, chemicals, or other unwanted substances) to prevent disease transmission or integumentary harm, typically using soap, water, and friction. An alcohol-based hand rub solution may be appropriate hand hygiene for hands not visibly soiled. (Chapter 4.1)
HCO3: Bicarbonate level reflected in arterial blood gas results. Normal range is 22-26 meq/L. (Chapter 11.2)
Healthcare-Associated Infections (HAIs): Unintended infections caused by care received in a health care setting. (Chapter 4.1)
Health history: The process of using directed interview questions to obtain symptoms and perceptions about a patient’s illness or life condition. The purpose of obtaining a health history is to gather subjective data from the patient and/or the patient’s family so that the health care team and the patient can collaboratively create a plan that will promote health, address acute health problems, and minimize chronic health conditions. (Chapter 2.1)
Heave or lift: Palpable lifting sensation under the sternum and anterior chest wall to the left of the sternum; it suggests severe right ventricular hypertrophy. (Chapter 9.3)
Hematemesis: Blood-tinged mucus secretions from the lungs. (Chapter 12.3)
Hematochezia: Passage of bloody stool. (Chapter 12.3)
Hematoma: An area of blood that collects outside of larger blood vessels. (Chapter 7.2, Chapter 20.2)
Hemoptysis: Blood-tinged mucus secretions from the lungs. (Chapter 10.3)
Hemosiderin staining: Dark-colored discoloration of the lower legs due to blood pooling. (Chapter 20.2)
Hemostasis phase: The first phase of wound healing that occurs immediately after skin injury. Blood vessels constrict and clotting factors are activated. (Chapter 20.2)
Hyperactive bowel sounds: Increased peristaltic activity; may be related to diarrhea, obstruction, or digestion of a meal. (Chapter 12.3)
Hypercapnia: Elevated carbon dioxide levels in the blood, indicated by PaCO2 level greater than 45 in an ABG test. (Chapter 10.3, Chapter 11.2)
Hyperglycemia: Elevated blood glucose reading with associated signs and symptoms such as frequent urination and increased thirst. (Chapter 19.2)
Hypertension: Elevated blood pressure over 130/80 mmHg in an adult. (Chapter 3.2)
Hypertonic solutions: Solutions that have a higher concentration of dissolved particles than blood. (Chapter 23.2)
Hypoactive bowel sounds: Decreased peristaltic activity; may be related to constipation following abdominal surgery or with an ileus. (Chapter 12.3)
Hypodermis: The layer of skin beneath the dermis composed of connective tissue and used for fat storage. (Chapter 14.2)
Hypoglycemia: A blood glucose reading less than 70 associated with symptoms such as irritability, shakiness, hunger, weakness, or confusion. If not rapidly treated, hypoglycemia can cause seizures and a coma. (Chapter 19.2)
Hypotension: Decreased blood pressure less than 90/60 mmHg in an adult. (Chapter 3.2)
Hypothalamus: The autonomic control center of the brain that controls functions such as blood pressure, heart rate, digestive movement, and pain perception. (Chapter 6.2)
Hypotonic solutions: Solutions that have a lower concentration of dissolved solutes than blood. (Chapter 23.2)
Hypoxemia: Decreased dissolved oxygen in the arterial blood, indicated by a PaO2 level less than 80 mmHg in an ABG test. (Chapter 10.3, Chapter 11.2)
Hypoxia: A reduced level of tissue oxygenation. (Chapter 11.2)

Ileostomy: The lower end of the small intestine (ileum) is attached to a stoma to bypass the colon, rectum, and anus. (Chapter 21.7)
Incident report: A report submitted per agency policy used to document the events surrounding a medication error. (Chapter 15.2)
Induration: Area of hardened tissue. (Chapter 20.3)
Indwelling catheter: A device often referred to as a “Foley catheter” that is inserted into the neck of the bladder and remains in place for continual collection of urine into a collection bag. (Chapter 21.2)
Infiltration: Infiltration occurs when the tip of the IV catheter slips out of the vein, the catheter passes through the wall of the vein, or the blood vessel wall allows part of the fluid to infuse into the surrounding tissue, resulting in the leakage of IV fluids into the surrounding tissue. (Chapter 23.4)
Inflammatory phase: The second phase of wound healing when vasodilation occurs so that white blood cells in the bloodstream can move into the wound to start cleaning the wound bed. (Chapter 20.2)
Inner cannula: The cannula inside the outer cannula that is removed during tracheostomy care by the nurse. Inner cannulas can be disposable or reusable with appropriate cleaning. (Chapter 22.2)
Inspection: The observation of a patient’s anatomical structures. (Chapter 1.1)
Instrumental activities of daily living: Complex daily tasks that allow patients to function independently such as managing finances, paying bills, purchasing and preparing meals, managing one’s household, taking medications, and facilitating transportation. (Chapter 2.8)
Intermittent catheterization: The insertion and removal of a straight catheter for relief of urinary retention. (Chapter 21.2)
Intradermal injection: Medication administered in the dermis just below the epidermis. (Chapter 18.1)
Intramuscular injection: Medication administered into a muscle. (Chapter 18.1)
Intravenous injection: Medication administered directly into the bloodstream. (Chapter 18.1)
Intravenous therapy (IV therapy): Involves the administration of substances such as fluids, electrolytes, blood products, nutrition, or medications directly into a client's vein. (Chapter 23.2)
Inunction: A medication that is massaged or rubbed into the skin. (Chapter 16.2)
Involuntary guarding: The reflexive contraction of overlying abdominal muscles as the result of peritoneal inflammation. (Chapter 12.3)
Iris: Colored part of the eye. (Chapter 8.2)
Isotonic solutions: IV fluids that have a similar concentration of dissolved particles as found in the blood. (Chapter 23.2)
J
Jaundice: A yellowing of the skin or sclera caused by underlying medical conditions. (Chapter 14.3, Chapter 14.4)
Joints: The location where bones come together. (Chapter 13.2)
Jugular Vein Distension (JVD): Occurs when the increased pressure of the superior vena cava causes the jugular vein to bulge, making it most visible on the right side of a person’s neck. (Chapter 9.3)
K
Keloid: A raised scar caused by overproduction of scar tissue. (Chapter 14.3)
Ketoacidosis: A life-threatening complication of hyperglycemia that can occur in patients with type 1 diabetes mellitus that is associated with symptoms such as fruity-smelling breath, nausea, vomiting, severe thirst, and shortness of breath. (Chapter 19.2)
Key part: Any sterile part of equipment used during an aseptic procedure, such as needle hubs, syringe tips, dressings, etc. (Chapter 4.3)
Key site: The site contacted during an aseptic procedure, such as nonintact skin, a potential insertion site, or an access site used for medical devices connected to the patients. Examples of key sites include the insertion or access site for intravenous (IV) devices, urinary catheters, and open wounds. (Chapter 4.3)
Kinesthesia: A person’s sense of movement. (Chapter 6.2)
Korotkoff sounds: The audible sounds of blood pressure named after Dr. Korotkoff who discovered them. (Chapter 3.2)
Kyphosis: Outward curvature of the back; often described as “hunchback.” (Chapter 10.3, Chapter 13.3)

Lacrimal duct: Tears produced by the lacrimal gland flow through this duct to the medial corner of the eye. (Chapter 8.2)
Laryngopharynx: The portion of the pharynx inferior to the oropharynx and posterior to the larynx that is a passageway for ingested material and air until its inferior end where the digestive and respiratory systems diverge into the esophagus and the larynx. (Chapter 7.2)
Larynx: The structure connecting the pharynx to the trachea that helps regulate the volume of air that enters and leaves the lungs and contains the vocal cords. (Chapter 7.2)
Lens: An inner part of the eye that helps the eye focus. (Chapter 8.2)
Lesion: An area of abnormal tissue. (Chapter 14.2)
Level of consciousness: A patient’s level of arousal and alertness, commonly assessed by asking them to report their name, current location, and time. (Chapter 6.4)
LGBTQ: An acronym standing for lesbian, gay, bisexual, transgender, and queer is an umbrella term that generally refers to a group of people who are diverse with regard to their gender identity and sexual orientation. There are expanded versions of this acronym. (Chapter 2.8)
Ligaments: Strong bands of fibrous connective tissue that connect bones and strengthen and support joints by anchoring bones together and preventing their separation. (Chapter 13.2)
Lordosis: An inward curve of the lumbar spine just above the buttocks. A small degree of lordosis is normal, but too much curving is called swayback. (Chapter 13.3)
Lymphadenopathy: Enlarged lymph nodes. (Chapter 7.3)
Lymphedema: A type of swelling that occurs when lymph fluid builds up in the body’s soft tissues due to damage to the lymph system. (Chapter 14.3)
Lymph nodes: Structures in the lymphatic system that filter pathogens. (Chapter 7.2)
M
Maceration: The softening and wasting away of skin due to excess fluid. (Chapter 20.2)
Macrodrip tubing: Gravity IV tubing with drop factors of 10, 15, or 20 drops per milliliter that are typically used to deliver general IV solutions for adults. (Chapter 5.14)
Main health care needs: Term used to classify what needs the patient feels are most important to address after admission to a health care agency. (Chapter 2.5)
Mandible: Lower jawbone. (Chapter 7.2)
MAR: Medication administration record contained in a patient’s chart. (Chapter 15.2)
Masseter: Main muscle used for chewing because it elevates the mandible to close the mouth. (Chapter 7.2)
Maturation phase: The final phase of wound healing as collagen continues to be created to strengthen the wound, causing scar tissue. (Chapter 20.2)
Maxilla: Bone that forms the upper jaw and supports the upper teeth. (Chapter 7.2)
Medical asepsis: Measures to prevent the spread of infection in health care agencies. (Chapter 1.2)
Medication cup: A small plastic or paper cup used to dispense oral medications. Some plastic medication cups have calibration marks for measuring medication amounts. (Chapter 5.2)
Medication reconciliation: A comparison of a list of current medications with a previous list and is completed at every hospitalization and clinic visit. (Chapter 2.6)
Melanin: Skin pigment produced by melanocytes scattered throughout the epidermis. (Chapter 14.2)
Melanoma: Skin cancer characterized by the uncontrolled growth of melanocytes that commonly develops from a mole. Melanoma is the most fatal of all skin cancers because it is highly metastatic. Melanomas usually appear as asymmetrical brown and black patches with uneven borders and a raised surface. (Chapter 14.3)
Melena: Dark, tarry-looking stool due to the presence of digested blood. (Chapter 2.8, Chapter 12.3)
Mental health: A state of well-being in which every individual realizes their own potential, can cope with normal stresses of life, can work productively and fruitfully, and is able to make a contribution to their community. (Chapter 2.8)
Metered dose inhaler (MDI): An inhaler that provides a mist of medication that is inhaled through the mouth into the lungs. An example of an MDI is albuterol. Optimal administration is achieved with a spacer attached to the inhaler. (Chapter 16.2)
Microdrip tubing: Gravity IV tubing with a drop factor of 60 drops per milliliter. (Chapter 5.14)
Midline peripheral catheters: A larger catheter (i.e., 16-18 gauge) that allow for rapid infusions but does not terminate in the central vasculature. (Chapter 23.2)
Military time: A method of measuring the time based on the full 24 hours of the day rather than two groups of 12 hours indicated by AM and PM. (Chapter 5.3)
Mobility: A patient’s ability to move around (e.g., sit up, sit down, stand up, walk). (Chapter 2.8)
Motor nerves: Nerves in the peripheral nervous system that transmit motor signals from the brain to the muscles to cause movement. (Chapter 6.2)
Murmur: A blowing or whooshing sound heard on auscultation of the precordium that signifies turbulent blood flow in the heart often caused by a valvular defect. (Chapter 9.3)
Muscle atrophy: The thinning or loss of muscle tissue that can be caused by disuse, aging, or neurological damage. (Chapter 13.2)
Myopia: Impaired vision, also known as nearsightedness, that makes far-away objects look blurry. (Chapter 8.2)
N
Nares: Nostril openings into the nasal cavity. (Chapter 7.2)
Nasal septum: Bone and cartilage that separate the nasal cavity into two compartments. (Chapter 7.2)
Nasopharynx: The upper region of the pharynx that connects to the nasal cavity and is a passageway for air. (Chapter 7.2)
Necrosis: Tissue death. (Chapter 23.2)
Necrotic: Black tissue color due to tissue death from lack of oxygenation to the area. (Chapter 20.2)
Nonblanchable erythema: Skin redness that does not turn white when pressure is applied. (Chapter 20.2)

One-time order: A prescription for a medication to be administered only once. An example of a one-time order is a prescription for an IV dose of antibiotics to be administered immediately prior to surgery. (Chapter 15.2)
Open fracture: A type of fracture when the broken bone punctures the skin. (Chapter 13.3)
Optic nerve: Cranial nerve II that conducts visual information from the retina to the brain. (Chapter 8.2)
Oral syringe: A specific type of syringe used to measure and/or administer medications via the oral route. (Chapter 5.2)
Orbit: The bony socket that houses the eyeball and muscles that move the eyeball. (Chapter 7.2)
Oropharyngeal suctioning: Suction of secretions through the mouth, often using a Yankauer device. (Chapter 22.4)
Oropharynx: The part of the throat at the back of the mouth behind the oral cavity. It includes the back third of the tongue, the soft palate, the side and back walls of the throat, and the tonsils. (Chapter 7.2, Chapter 19.4)
Orthopnea: Breathlessness or a feeling of shortness of breath when lying in a reclined position. (Chapter 9.3, Chapter 10.3)
Orthostatic hypotension: A decrease in blood pressure by at least 20 mmHg systolic or 10 mmHg diastolic within three minutes of standing from a seated or lying position. (Chapter 3.2)
Osteoarthritis: The most common type of arthritis associated with aging and wear and tear of the articular cartilage that covers the surfaces of bones at the synovial joint. (Chapter 13.3)
Osteomyelitis: Bone infection. (Chapter 20.2)
Osteoporosis: A disease that thins and weakens bones, especially in the hip, spine, and wrist, causing them to become fragile and break easily. (Chapter 13.3)
Ostomy: The surgical procedure that creates the opening from the stoma outside the body to an organ such as the small intestine, colon, rectum, or bladder. A stoma can be permanent, such as when an organ is removed, or temporary, such as when an organ requires time to heal. (Chapter 21.7)
Otitis externa: The medical diagnosis for external ear inflammation and/or infection. (Chapter 8.2)
Ototoxic medications: Medications that cause the adverse effect of sensorineural hearing loss by affecting the hair cells in the cochlea. (Chapter 8.2)
Outer cannula: The outer cannula placed by the provider through the tracheostomy stoma and continuously remains in place. (Chapter 22.2)
P
Pallor: A reduced amount of oxyhemoglobin in the skin or mucous membranes. Skin and mucous membranes present with a pale skin color. (Chapter 9.3, Chapter 10.3, Chapter 14.4)
PaO2: The partial pressure of dissolved oxygen in the blood measured by arterial blood gas samples. (Chapter 11.2)
Paralysis: The partial or complete loss of strength, movement, or control of a muscle or group of muscles within a body part that can be caused by brain or spinal injury. (Chapter 6.5)
Paroxysmal nocturnal dyspnea: An attack of severe shortness of breath that generally occurs at night. (Chapter 9.3)
Passive range of motion: The degree of range of motion a patient demonstrates in a joint when the examiner is providing the movement. (Chapter 13.4)
Percussion: An advanced physical examination technique where body parts are tapped with fingers to determine their size and if fluid is present. (Chapter 1.1)
Perfusion: The amount of blood flow to tissue. (Chapter 9.3)
Pericardial friction rub: Uncommon heart sounds produced when the parietal and visceral pericardium become inflamed, generating a creaky-scratchy noise as they rub together. (Chapter 9.3)
Peripheral edema: Swelling due to an accumulation of fluid in tissues perfused by the peripheral vascular system. (Chapter 9.3)
Peripheral inserted central catheter: A thin, flexible tube inserted into a vein in the upper arm and guided into the superior vena cava. (Chapter 23.2)
Peripheral IV: An intravenous catheter inserted by percutaneous venipuncture into a peripheral vein and held in place with a sterile transparent dressing. (Chapter 23.2)
Peripheral nervous system: The part of the nervous system that includes the cranial and spinal nerves. (Chapter 6.2)
Peripheral neuropathy: A condition that causes decreased sensation of pain and pressure, typically in the lower extremities. (Chapter 20.2)
Periwound: The skin around the outer edges of a wound. (Chapter 20.3)
Personal Protective Equipment (PPE): Includes gloves, gowns, goggles, face shields, and masks, along with environmental controls, to prevent the transmission of infection for patients who are diagnosed or suspected of having an infectious disease. (Chapter 1.2, Chapter 4.1)
Petechiae: Tiny red dots caused by bleeding under the skin. (Chapter 14.3)
Pharyngitis: Infection and/or inflammation in the back of the throat (pharynx). (Chapter 7.3)
Pharynx: A tube lined with mucous membrane that begins at the nasal cavity and is divided into three major regions: the nasopharynx, the oropharynx, and the laryngopharynx. (Chapter 7.2)
Phlebitis: Inflammation of a vein. (Chapter 23.4)
Physical examination: A systematic data collection method of the body that uses the techniques of inspection, auscultation, palpation, and percussion. (Chapter 1.1)
Pitting edema: An accumulation of fluid in tissue and causes an indentation when the area is pressed. (Chapter 9.3)
Pleural rub: Uncommon sound heard when there is inflammation of the lung pleura, resulting in friction as the surfaces rub against each other. (Chapter 10.3)
Precordium: The region of the thorax in front of the heart. (Chapter 9.3)
Presbycusis: Sensorineural hearing loss that occurs with aging due to gradual nerve degeneration. (Chapter 8.2)
Presbyopia: Impaired near vision that commonly occurs in middle-aged and older adults. (Chapter 8.2)
Prescriptions: Orders, interventions, remedies, or treatments ordered or directed by an authorized primary health care provider. (Chapter 15.2)
Pressure injury: Skin breakdown caused when a patient’s skin and soft tissue press against a hard surface for a prolonged period of time, causing reduced blood supply and resulting in damaged tissue. (Chapter 14.3, Chapter 20.2)
Primary intention: Wound healing that occurs with surgical incisions or clean-edged lacerations that are closed with sutures, staples, or surgical glue. (Chapter 20.2)
Primary source of data: Information obtained directly from the patient. (Chapter 1.1, Chapter 2.2)
Primary survey: A brief observation at the start of a shift or visit to verify the patient is stable by assessing mental status, airway, breathing, and circulation. (Chapter 1.4)
PRN order: A prescription for medication to be administered when it is requested by, or as needed by, the patient. PRN orders are usually administered based on patient symptoms such as pain medications. An example of a PRN order is a prescription for pain medication, such as “Acetaminophen 500 mg PO every 4-6 hours as needed for pain.” (Chapter 15.2)
Proliferative phase: The third phase of wound healing that includes epithelialization, angiogenesis, collagen formation, and contraction. (Chapter 20.2)
Proprioception: A person’s sense of their body position. (Chapter 6.2, Chapter 6.8)
Prostate hypertrophy: A common medical condition of the enlargement of the prostate gland in males as they age, causing uncomfortable urinary symptoms such as urgency and frequency. (Chapter 12.3)
Protuberant: Convex or bulging appearance. (Chapter 12.3)
Ptosis: Drooping of the eyelid. (Chapter 6.5)
Pulmonary embolism: A blood clot that lodges in one of the arteries that go from the heart to the lung. (Chapter 9.3)
Pupil: The hole at the center of the eye that allows light to enter. (Chapter 8.2)
Purulent drainage: Wound exudate that is thick and opaque and can be tan, yellow, green, or brown in color. It is never considered normal in a wound, and new purulent drainage should always be reported to the health care provider. (Chapter 20.3)

Rales: Another term used for crackles. (Chapter 10.3)
Rebound tenderness: Pain when hand is withdrawn during palpation. (Chapter 12.3)
Reconstitution: The process of adding a liquid diluent to a dry ingredient to make a liquid in a specific concentration. (Chapter 5.10)
Respiration: Includes ventilation and gas exchange at the alveolar level where blood is oxygenated and carbon dioxide is removed. (Chapter 10.2)
Retina: The nervous tissue and photoreceptors in the eye that initially process visual stimuli. (Chapter 8.2)
Retractions: The “pulling in” of muscles between the ribs or in the neck when breathing, indicating difficulty breathing or respiratory distress. (Chapter 10.3)
Rheumatoid arthritis: A type of arthritis that causes pain, swelling, stiffness, and loss of function in joints due to inflammation caused by an autoimmune disease. (Chapter 13.3)
RICE: A mnemonic for treatment of sprains and strains that stands for: Resting the injured area, Icing the area, Compressing the area with an ACE bandage or other device, and Elevating the affected limb. (Chapter 13.3)
Rigidity: Involuntary contraction of the abdominal musculature in response to peritoneal inflammation. (Chapter 12.3)
Rotation: Circular movement of a joint around a fixed point. (Chapter 13.2)
Routine order: A written prescription that is followed until another order cancels it. An example of a routine order is a prescription for daily medication such as “Lisinopril 10 mg PO daily.” (Chapter 15.2)
Rule of Nines: A tool used in the emergency department to assess the total body surface area burned to quickly estimate intravenous fluid requirements. (Chapter 14.3)

Safety culture: A culture established in health care agencies to empower staff to speak up about risks to patients and to report errors and near misses, all of which drive improvement in patient care and reduce the incident of patient harm. (Chapter 15.2)
Saline lock: Refers to the use of a short extension set that allows IV access without requiring ongoing IV infusions. (Chapter 23.2)
Sanguineous drainage: Wound drainage that is fresh bleeding. (Chapter 20.3)
SaO2: Calculated arterial oxygen saturation level. (Chapter 11.2)
Scaphoid: Sunken appearance. (Chapter 12.3)
Sclera: White area of the eye. (Chapter 8.2)
Scoliosis: A sideways curve of the spine that commonly develops in late childhood and the early teens. (Chapter 13.3)
Secondary intention: Wound healing that occurs when the edges of a wound cannot be approximated (brought together), so the wound fills in from the bottom up by the production of granulation tissue. Examples of wounds that heal by secondary intention are pressure injuries and chainsaw injuries. (Chapter 20.2)
Secondary source of data: Information from the patient’s chart, family members, or other health care team members. (Chapter 1.1, Chapter 2.2)
Second-degree burn: Burn affecting both the epidermis and a portion of the dermis, resulting in swelling and a painful blistering of the skin. (Chapter 14.3)
Self-concept: Knowledge a person has about themselves that makes up who they are (i.e., their identity). (Chapter 2.8)
Self-esteem: A person’s self-evaluation of their self-concept as being worthy or unworthy. (Chapter 2.8)
Sensation: The function of receiving information about the environment. The major senses are taste, smell, touch, sight, and hearing. (Chapter 6.2)
Sensorineural hearing loss: Hearing loss caused by pathology of the inner ear, cranial nerve VIII, or auditory areas of the cerebral cortex. (Chapter 8.2)
Sensory nerves: Nerves in the peripheral nervous system that carry impulses from the body to the brain for processing. (Chapter 6.2)
Serosanguinous drainage: Wound exudate contains serous drainage with small amounts of blood present. (Chapter 20.3)
Serous drainage: Wound drainage that is clear, thin, watery plasma. It is considered normal in minimal amounts during the inflammatory stage of wound healing. (Chapter 20.3)
Sexual orientation: The preferred term used when referring to an individual’s physical and/or emotional attraction to the same and/or opposite gender. (Chapter 2.8)
Shear: A mechanical force that occurs when tissue layers move over the top of each other, causing blood vessels to stretch and break as they pass through the subcutaneous tissue. (Chapter 20.2)
Sign: Objective data found by the nurse or health care provider when assessing a patient. (Chapter 2.2)
Sinusitis: Inflamed sinuses caused by a viral or bacterial infection. (Chapter 7.3)
Skeletal muscle: Voluntary muscle that produces movement, assists in maintaining posture, protects internal organs, and generates body heat. (Chapter 13.2)
Skin tears: Wounds caused by mechanical forces, typically in the nonelastic skin of older adults. (Chapter 20.2)
Skin turgor: The skin’s elasticity and its ability to change shape and return to normal when gently grasped between two fingers. (Chapter 14.4)
Slough: Inflammatory exudate that is light yellow, soft, and moist and must be removed for wound healing to occur. (Chapter 20.2)
Small-volume nebulizers: Devices that provide a fine mist using oxygen or compressed air to transport the medication from a nebulizer cup into the mouth and into the lungs as the patient breathes normally through a mask or pipe device. (Chapter 16.2)
Snellen chart: A chart used to test far vision. (Chapter 8.2)
Sphygmomanometer: A device used to measure blood pressure and is commonly referred to as a blood pressure cuff. (Chapter 3.2, Chapter 9.3)
Spirituality: A way of living that comes from a set of meanings, values, and beliefs that are important to a person. (Chapter 2.8)
SpO2: An estimated oxygenation level based on the saturation level of hemoglobin measured by a pulse oximeter. (Chapter 11.2)
Sprain: A stretched or torn ligament caused by an injury. (Chapter 13.3)
Standardized sliding-scale insulin protocol: Standardized instructions for administration of adjustable insulin dosages based on a patient’s premeal blood glucose readings. (Chapter 19.2)
Standard precautions: The minimum infection prevention practices that apply to all patient care, regardless of suspected or confirmed infection status of the patient, in any setting where health care is delivered. (Chapter 4.2)
Standing order: Standing orders are standard prescriptions for nurses to implement for patients in clearly defined circumstances without the need to notify a provider. They may also be referred to as an “order set” or a “protocol.” An example of a standing order/protocol is a standard prescription for all patients coming into an urgent care reporting chest pain to immediately receive four chewable aspirin, the placement of an IV, and an electrocardiogram (ECG). (Chapter 15.2)
STAT order: A one-time prescription that is administered without delay. An example of a STAT order is the prescription for a dose of Benadryl to be administered to a patient having an allergic reaction. (Chapter 15.2)
Stereognosis: The ability to perceive the physical form and identity of an object based on tactile stimuli alone. (Chapter 6.6)
Sterile technique (surgical asepsis): Techniques used to eliminate every potential microorganism in and around a sterile field while maintaining objects and areas as free from microorganisms as possible. This technique is the standard of care for surgical procedures, invasive wound management, and central line care. (Chapter 4.3)
Sternocleidomastoid: The major muscle that laterally flexes and rotates the head. (Chapter 7.2)
Stoma: An opening on the abdomen that is connected to the gastrointestinal or urinary systems to allow waste (urine or feces) to be collected in a pouch. (Chapter 21.7)
Stomach decompression: Using suctioning through a nasogastric tube to remove the contents of the stomach. (Chapter 17.1)
Straight catheter: A catheter used for intermittent urinary catheterization; it does not have a balloon at the insertion end. (Chapter 21.2)
Strain: A stretched or torn muscle or tendon. (Chapter 13.3)
Striae: White or silver markings from stretching of the skin. (Chapter 12.3)
Stridor: High-pitched crowing sounds heard over the upper airway and larynx indicating obstruction. (Chapter 10.3)
Subcutaneous injection: Medication administered into the subcutaneous tissue just under the dermis. (Chapter 18.1)
Subjective data: Information obtained from the patient and/or family members that offers important cues from their perspectives. (Chapter 1.1, Chapter 2.2)
Suction canister: A container for collecting suctioned secretions that is attached to a suction source. (Chapter 22.4)
Suction catheter: A soft, flexible, sterile catheter used for nasopharyngeal and tracheostomy suctioning. (Chapter 22.4)
Suture: An interlocking joint between adjacent bones of the skull. (Chapter 7.2)
Symptom: Subjective data that the patient reports, such as “I feel dizzy.” (Chapter 2.2)
Syncope: A temporary loss of consciousness usually related to insufficient blood flow to the brain. (Chapter 9.3)
Synovial fluid: A thick fluid that provides lubrication in joints to reduce friction between the bones. (Chapter 13.2)
Synovial joints: A fluid-filled joint cavity where the articulating surfaces of the bones contact and move smoothly against each other. The elbow and knee are examples of synovial joints. (Chapter 13.2)
Syringe: A medical device used to administer parenteral medication into tissue or into the bloodstream. Syringes can also be used to withdraw blood or fluid. (Chapter 5.2)
Systole: The phase of the heartbeat when the left ventricle contracts and pumps blood into the arteries. (Chapter 3.2)
Systolic blood pressure: The maximum pressure of blood on the arteries during the contraction of the left ventricle of the heart referred to as systole. (Chapter 3.2)

Tachypnea: Rapid and often shallow breathing greater than normal range according to the patient’s age. (Chapter 10.3)
Temporalis: Muscle that assists in chewing by retracting the mandible. The temporalis muscle can be felt moving by placing fingers on the patient’s temple as they chew. (Chapter 7.2)
Tendons: Strong bands of dense, regular connective tissue that connect muscles to bones. (Chapter 13.2)
Tertiary intention: Wound healing that occurs when a wound must remain open or has been reopened, often due to severe infection. (Chapter 20.2)
Thalamus: Relays sensory information and motor information in collaboration with the cerebellum. (Chapter 6.2)
Third-degree burn: Severe burn that fully extends into the epidermis and dermis, destroying the tissue and affecting the nerve endings and sensory function. (Chapter 14.3)

Total parenteral nutrition (TPN): A very concentrated solution that must be administered via a central line. (Chapter 23.2)
Trachea: A tube lined with mucus membrane that carries air from the larynx to the lungs. (Chapter 7.2)
Tracheostomy: A surgically created opening that goes from the front of the neck into the trachea. (Chapter 22.2)
Tracheostomy dressing: A manufactured dressing used with tracheostomies that does not shed fibers, which could potentially be inhaled by the patient. (Chapter 22.4)
Transdermal route: Patches or disks applied to the skin that deliver medication over an extended period of time. (Chapter 16.2)
Trapezius: The muscle that elevates the shoulders (shrugs), pulls the shoulder blades together, and tilts the head backwards. (Chapter 7.2)
Tunneling: Passageways underneath the surface of the skin that extend from a wound and can take twists and turns. (Chapter 20.2)
Tympanic membrane: The membrane at the end of the external ear canal, commonly called the eardrum, that vibrates after it is struck by sound waves. (Chapter 8.2)

Undermining: A condition that occurs in wounds when the tissue under the wound edges becomes eroded, resulting in a pocket beneath the skin at the wound’s edge. (Chapter 20.2)
Unstageable: Occurs when slough or eschar obscures the wound so that tissue loss cannot be assessed. (Chapter 20.2)
Urinary catheterization: The insertion of a catheter tube into the urethral opening and placing it in the neck of the urinary bladder to drain urine. (Chapter 21.2)
Urinary frequency: Urination every hour or two. (Chapter 12.3)
Urinary incontinence: Involuntary leakage of urine. (Chapter 12.3)
Urinary tract infection (UTI): An infection in the urinary system causing symptoms such as burning on urination (dysuria), frequent urination, malodorous urine, fever, and change in level of consciousness. (Chapter 21.2)
Urinary urgency: An intense urge to urinate that can lead to urinary incontinence. (Chapter 12.3)
Urostomy: The ureters (tubes that carry urine from the kidney to the bladder) are attached to a stoma to bypass the bladder. (Chapter 21.7)
Uvula: A small, teardrop-shaped structure located at the apex of the soft palate that swings upward during swallowing to close off the nasopharynx and prevent ingested materials from entering the nasal cavity. (Chapter 7.2)
V
Value: An accepted principle or standard of an individual or group. (Chapter 2.8)
Vastus lateralis: A muscle located on the anterior lateral aspect of the thigh and extends from one hand’s breadth above the knee to one hand’s breadth below the greater trochanter. It is commonly used for immunizations in infants and toddlers because the muscle is thick and well-developed. (Chapter 18.6)
Venipuncture: The process of introducing a needle into a client’s vein to collect a blood sample or insert an IV catheter. (Chapter 23.2)
Venous insufficiency: A medical condition where the veins in the legs do not adequately send blood back to the heart, resulting in a pooling of fluids in the legs that can cause venous ulcers. (Chapter 20.2)
Venous ulcers: Ulcers caused by the pooling of fluid in the veins of the lower legs when the valves are not working properly, causing fluid to seep out, macerate the skin, and cause an ulcer. (Chapter 20.2)
Ventilation: The mechanical movement of air into and out of the lungs. (Chapter 10.2, Chapter 11.3)
Ventrogluteal: The safest intramuscular injection site for adults and children because it provides the greatest thickness of gluteal muscles, is free from penetrating nerves and blood vessels, and has a thin layer of fat. (Chapter 18.6)
Vertigo: A type of dizziness often described by patients as “the room feels as if it is spinning.” (Chapter 8.2)
Vesicular sounds: Low-pitched soft sounds like “rustling leaves” heard over alveoli and small bronchial airways. (Chapter 10.3)
Vestibulocochlear nerve: Cranial nerve VIII that transports neural signals from the cochlea and the vestibule to the brain stem regarding hearing and balance. (Chapter 8.2)
Voiding: Medical terminology used for urinating. (Chapter 2.8)

Diagnosis is the second step of the nursing process (and the second Standard of Practice by the American Nurses Association). This standard is defined as, "The registered nurse analyzes assessment data to determine actual or potential diagnoses, problems, and issues." The RN “prioritizes diagnoses, problems, and issues based on mutually established goals to meet the needs of the health care consumer across the health–illness continuum and the care continuum.” Diagnoses, problems, strengths, and issues are documented in a manner that facilitates the development of expected outcomes and a collaborative plan.[1] See Figure 4.7a for an illustration of how the Diagnosis phase of the nursing process corresponds to the NCSBN Clinical Judgment Measurement Model (NCJMM).[2]
Analyzing Assessment Data
After collection of assessment data, the RN analyzes the data to form generalizations and create and prioritize hypotheses for nursing diagnoses. Steps for analyzing assessment data include performing data analysis, clustering information, identifying hypotheses for potential nursing diagnosis, performing additional in-depth assessment as needed, and establishing nursing diagnosis statements. The nursing diagnoses are then prioritized and the nursing care plan is developed based on them.[3] Analyzing assessment data is completed by an RN and falls outside of the scope of practice of the LPN/VN. However, LPN/VNs must understand data analysis so that new, concerning data is promptly reported to the RN for follow-up.
Performing Data Analysis
After nurses collect assessment data from a client, they use their nursing knowledge to analyze that data to determine if it is “expected” or “unexpected” or “normal” or “abnormal” for that client according to their age, development, and baseline status. From there, nurses determine what data is “clinically relevant” as they prioritize their nursing care.[4]
Example of Analyzing Cues
In Scenario C in the "Assessment" section of this chapter, the nurse analyzes the vital signs data and determines the blood pressure, heart rate, and respiratory rate are elevated, and the oxygen saturation is decreased for this client. These findings are considered “relevant cues" because they are abnormal compared to this client's baseline and may indicate a new health problem or complication is occurring.
Clustering Information/Seeing Patterns/Making Hypotheses
After analyzing the data and determining relevant cues, the nurse begins clustering data into similar domains or patterns. Evidence-based assessment frameworks, such as Gordon’s Functional Health Patterns, assist nurses in clustering data based on patterns of human responses. See the box below for an outline of Gordon’s Functional Health Patterns.[5] Concepts related to many of these patterns will be discussed in chapters later in this book.

Example of Using Gordon's Health Patterns to Cluster Data
Refer to Scenario C in the "Assessment" section of this chapter. The nurse clusters the following relevant cues: elevated blood pressure, elevated respiratory rate, crackles in the lungs, weight gain, worsening edema, shortness of breath, medical history of heart failure, and currently prescribed a diuretic medication into a pattern of fluid balance, which can be classified under Gordon’s Nutritional-Metabolic Functional Health Pattern. Based on the related data in this cluster, the nurse makes a hypothesis that the client has excess fluid volume present.
Identifying Nursing Diagnoses
After the nurse has analyzed and clustered the data from the client assessment, the next step is to begin to answer the question, “What are my client’s human responses to their health condition(s) (i.e., their nursing diagnoses)?” A nursing diagnosis is defined as, "A clinical judgment concerning a human response to health conditions/life processes, or a vulnerability for that response, by an individual, family, group, or community."[7] Nursing diagnoses are customized to each client and drive the development of the nursing care plan. The nurse should refer to a care planning resource and review the definitions and defining characteristics of the hypothesized nursing diagnoses to determine if additional in-depth assessment is needed before selecting the most accurate nursing diagnosis. Formulation of nursing diagnoses is completed by an RN and is outside the scope of practice of LPN/VNs.
Nursing diagnoses are developed by nurses, for use by nurses. For example, NANDA International (NANDA-I) is a global professional nursing organization that develops nursing terminology that names actual or potential human responses to health problems and life processes based on research findings.[8] Currently, there are over 220 NANDA-I nursing diagnoses developed by nurses around the world. This list is continuously updated, with new nursing diagnoses added and old nursing diagnoses retired that no longer have supporting evidence. A list of commonly used NANDA-I diagnoses is listed in Appendix A. For a full list of NANDA-I nursing diagnoses, refer to a current nursing care plan reference.
NANDA-I nursing diagnoses are grouped into 13 domains that assist the nurse in selecting diagnoses based on the patterns of clustered data. These domains are similar to Gordon’s Functional Health Patterns and include health promotion, nutrition, elimination and exchange, activity/rest, perception/cognition, self-perception, role relationship, sexuality, coping/stress tolerance, life principles, safety/protection, comfort, and growth/development.
NANDA Diagnoses and the NCLEX
Knowledge regarding specific NANDA-I nursing diagnoses is not assessed on the NCLEX. However, analyzing cues, clustering data, forming appropriate hypotheses, and prioritizing hypotheses are components of clinical judgment assessed on the NCLEX and used in nursing practice. Nursing Diagnoses vs. Medical Diagnoses
You may be asking yourself, “How are nursing diagnoses different from medical diagnoses?” Medical diagnoses focus on diseases or other medical problems that have been identified by the physician, physician’s assistant, or advanced nurse practitioner. Nursing diagnoses focus on the human response to health conditions and life processes and are made independently by RNs. Clients with the same medical diagnosis will often respond differently to that diagnosis and thus have different nursing diagnoses. For example, two clients have the same medical diagnosis of heart failure. However, one client may be interested in learning more information about the condition and the medications used to treat it, whereas another client may be experiencing anxiety when thinking about the effects this medical diagnosis will have on their family. The nurse must consider these different responses when creating the nursing care plan. Nursing diagnoses consider the client’s and family’s needs, attitudes, strengths, challenges, and resources as a customized nursing care plan is created to provide holistic and individualized care for each client.
Example of a Medical Diagnosis
A medical diagnosis identified for Ms. J. in Scenario C in the "Assessment" section is heart failure. This cannot be used as a nursing diagnosis because it is outside the nurse's scope of practice to make a medical diagnosis, but it is considered as an “associated condition” when creating hypotheses for nursing diagnoses. Associated conditions are medical diagnoses, injuries, procedures, medical devices, or pharmacological agents that are not independently modifiable by the nurse, but support accuracy in nursing diagnosis. The nursing diagnosis in Scenario C will relate to the client’s responses to her medical diagnosis of heart failure, such as "Excess Fluid Volume."
Additional Definitions Used in NANDA-I Nursing Diagnoses
The following definitions are used in association with NANDA-I nursing diagnoses.
Patient
The NANDA-I definition of a “patient” includes the following:
- Individual: a single human being distinct from others (i.e., a person).
- Caregiver: a family member or helper who regularly looks after a child or a sick, elderly, or disabled person.
- Family: two or more people having continuous or sustained relationships, perceiving reciprocal obligations, sensing common meaning, and sharing certain obligations toward others; related by blood and/or choice.
- Group: a number of people with shared characteristics, such as an ethnic group.
- Community: a group of people living in the same locale under the same governance. Examples include neighborhoods and cities.[9]
Age
The age of the person who is the subject of the diagnosis is defined by the following terms[10]:
- Fetus: an unborn human more than eight weeks after conception, until birth.
- Neonate: a person less than 28 days of age.
- Infant: a person greater than 28 days and less than 1 year of age.
- Child: a person aged 1 to 9 years.
- Adolescent: a person aged 10 to 19 years.
- Adult: a person older than 19 years of age unless national law defines a person as being an adult at an earlier age.
- Older adult: a person greater than 65 years of age.
Time
The duration of the diagnosis is defined by the following terms[11]:
- Acute: lasting less than three months.
- Chronic: lasting greater than three months.
- Intermittent: stopping or starting again at intervals.
- Continuous: uninterrupted, going on without stop.
Two terms used to assist in creating nursing diagnoses are at-risk populations and associated conditions[12]:
- At-risk populations are groups of people who share a characteristic that causes each member to be susceptible to a particular human response, such as demographics, health/family history, stages of growth/development, or exposure to certain events/experiences.
- Associated conditions are medical diagnoses, injuries, procedures, medical devices, or pharmacological agents. These conditions are not independently modifiable by the nurse, but support accuracy in nursing diagnosis.[13]
Types of Nursing Diagnoses
There are four types of NANDA-I nursing diagnoses:[14]
- Problem-Focused
- Health Promotion - Wellness
- Risk
- Syndrome
A problem-focused nursing diagnosis is a “clinical judgment concerning an undesirable human response to health condition/life processes that exist in an individual, family, group, or community.”[15] To make an accurate problem-focused diagnosis, related factors and defining characteristics must be present. Related factors (also called etiology) are causes that contribute to the diagnosis. Defining characteristics are cues, signs, and symptoms that cluster into patterns.[16] Defining characteristics are the signs and symptoms that a nurse can observe, hear, feel, or smell and cluster into patterns underlying nursing diagnoses.
A health promotion-wellness nursing diagnosis is “a clinical judgment concerning motivation and desire to increase well-being and to actualize human health potential.” These responses are expressed by the client’s readiness to enhance specific health behaviors.[17] A health promotion-wellness diagnosis is used when the client is willing to improve a lack of knowledge, coping, or other identified need.
A risk nursing diagnosis is “a clinical judgment concerning the vulnerability of an individual, family, group, or community for developing an undesirable human response to health conditions/life processes.”[18] A risk nursing diagnosis must be supported by risk factors that contribute to the increased vulnerability. A risk nursing diagnosis is different from the problem-focused diagnosis in that the problem has not yet actually occurred. Problem diagnoses should not be automatically viewed as more important than risk diagnoses because sometimes a risk diagnosis can have the highest priority for a client.[19]
A syndrome nursing diagnosis is a “clinical judgment concerning a specific cluster of nursing diagnoses that occur together and are best addressed together and through similar interventions.”[20]
Establishing Nursing Diagnosis Statements
NANDA-I recommends creating statements for nursing diagnosis that include the nursing diagnosis and related factors as exhibited by defining characteristics. The accuracy of the nursing diagnosis is validated when a nurse is able to clearly link the defining characteristics, related factors, and/or risk factors found during the client’s assessment.[21]
To create a nursing diagnosis statement, the RN analyzes the client’s subjective and objective data and clusters the data into patterns. Based on these patterns, the RN generates hypotheses for nursing diagnoses based on how the patterns meet defining characteristics of a nursing diagnosis. Recall that "defining characteristics" are the signs and symptoms related to a nursing diagnosis.[22] Defining characteristics are included in care planning resources for each nursing diagnosis, along with a definition of that diagnosis, so the nurse can select the most accurate diagnosis.
Example
An RN clusters objective and subjective data such as weight, height, and dietary intake as a pattern related to nutritional status and then compares these signs and symptoms to the defining characteristics for the NANDA nursing diagnosis, "Imbalanced Nutrition: Less Than Body Requirement."
When creating a nursing diagnosis statement, the nurse also identifies the cause, or etiology, of the problem for that specific client. Recall that the term "related factors" refers to the underlying causes (etiology) of a client’s problem or situation. Related factors should not refer to medical diagnoses, but instead should be causes that the nurse can treat. When possible, the nursing interventions planned for nursing diagnoses should attempt to modify or remove these underlying causes of the nursing diagnosis.[23]
Creating nursing diagnosis statements is also called “using PES format.” The PES mnemonic no longer applies to the current terminology used by NANDA-I, but the components of a nursing diagnosis statement remain the same. A nursing diagnosis statement should contain the problem, related factors, and defining characteristics. These terms fit under the former PES format in this manner:
Problem (P): The problem (i.e., the nursing diagnosis)
Etiology (E): The related factors (i.e., the etiology/cause) of the nursing diagnosis; phrased as “related to” or “R/T”
Signs and Symptoms (S): The defining characteristics manifested by the client (i.e., the signs and symptoms/subjective and objective data/clinical cues) that led to the identification of that nursing diagnosis/hypothesis for the client; phrased with “as manifested by" (AMB) or "as evidenced by" (AEB).
Examples of different types of nursing diagnoses are further explained in the following sections.
Problem-Focused Nursing Diagnosis
A problem-focused nursing diagnosis contains all three components of the PES format:
Problem (P): Client problem (nursing diagnosis)
Etiology (E): Related factors causing the nursing diagnosis
Signs and Symptoms (S): Defining characteristics/cues manifested by that client (i.e., the signs and symptoms demonstrating there is a problem)
Example of a Problem-Focused Nursing Diagnosis
Refer to Scenario C of the "Assessment" section of this chapter. The cluster of data for Ms. J. (elevated blood pressure, elevated respiratory rate, crackles in the lungs, weight gain, worsening edema, and shortness of breath) are defining characteristics for the NANDA-I Nursing Diagnosis Excess Fluid Volume. The NANDA-I definition of Excess Fluid Volume is “surplus intake and/or retention of fluid.” The related factor (etiology) of the problem is that the client has excessive fluid intake.[24]
The components of a problem-focused nursing diagnosis statement for Ms. J. would be:
Problem (P): Excess Fluid Volume
Etiology (E): Related to excessive fluid intake
Signs and Symptoms (S): As manifested by bilateral basilar crackles in the lungs, bilateral 2+ pitting edema of the ankles and feet, increased weight of 1ten pounds, and the client reports, “My ankles are so swollen.”
A correctly written problem-focused nursing diagnosis statement for Ms. J. would be written as follows:
Excess Fluid Volume related to excessive fluid intake as manifested by bilateral basilar crackles in the lungs, bilateral 2+ pitting edema of the ankles and feet, an increase weight of 1ten pounds, and the client reports, “My ankles are so swollen.”
Health-Promotion Nursing Diagnosis
A health-promotion nursing diagnosis statement contains the problem (P) and the defining characteristics (S). The defining characteristics component of a health-promotion nursing diagnosis statement should begin with the phrase “expresses desire to enhance,” followed by what the client states in relation to improving their health status:[25]
A health-promotion diagnosis statement consists of the following:
Problem (P): Client problem (nursing diagnosis)
Signs and Symptoms (S): The client’s expressed desire to enhance
Example of a Health-Promotion Nursing Diagnosis
Refer to Scenario C in the "Assessment" section of this chapter. Ms. J. demonstrates a readiness to improve her health status when she told the nurse that she would like to “learn more about my health so I can take better care of myself.” This statement is a defining characteristic of the NANDA-I nursing diagnosis Readiness for Enhanced Health Management, which is defined as “a pattern of regulating and integrating into daily living a therapeutic regimen for the treatment of illness and its sequelae, which can be strengthened.”[26]
The components of a health-promotion nursing diagnosis for Ms. J. would be:
Problem (P): Readiness for Enhanced Health Management
Symptoms (S): Expressed desire to “learn more about my health so I can take better care of myself.”
A correctly written health-promotion nursing diagnosis statement for Ms. J. would be written as follows:
Enhanced Readiness for Health Promotion as manifested by expressed desire to “learn more about my health so I can take better care of myself.”
Risk Nursing Diagnosis
A risk nursing diagnosis should be supported by evidence of the client’s risk factors for developing that problem. Different experts recommend different phrasing. NANDA-I 2018-2020 recommends using the phrase “as evidenced by” to refer to the risk factors for developing that problem.[27]
A risk diagnosis consists of the following:
Problem (P): Client problem (nursing diagnosis)
As Evidenced By: Risk factors for developing the problem
Example of a Risk Nursing Diagnosis
Refer to Scenario C in the "Assessment" section of this chapter. Ms. J. has an increased risk of falling due to vulnerability from the dizziness and weakness she is experiencing. The NANDA-I definition of Risk for Falls is “increased susceptibility to falling, which may cause physical harm and compromise health.”[28]
The components of a risk nursing diagnosis statement for Ms. J. would be:
Problem (P): Risk for Falls
As Evidenced By: Dizziness and decreased lower extremity strength
A correctly written risk nursing diagnosis statement for Ms. J. would be written as follows:
Risk for Falls as evidenced by dizziness and decreased lower extremity strength.
Syndrome Nursing Diagnosis
A syndrome nursing diagnosis statement is a cluster of nursing diagnoses that occur together and are best addressed together and through similar interventions. To create a syndrome diagnosis, two or more nursing diagnoses must be used as defining characteristics (S) that create a syndrome. Related factors may be used if they add clarity to the definition but are not required.[29]
A syndrome statement consists of these items:
Problem (P): The syndrome
Signs and Symptoms (S): The defining characteristics are two or more similar nursing diagnoses
Example of a Syndrome Nursing Diagnosis
Refer to Scenario C in the "Assessment" section of this chapter. Clustering the data for Ms. J. identifies several similar NANDA-I nursing diagnoses that can be categorized as a syndrome. For example, Activity Intolerance is defined as “insufficient physiological or psychological energy to endure or complete required or desired daily activities.” Social Isolation is defined as “aloneness experienced by the individual and perceived as imposed by others and as a negative or threatening state.” These diagnoses can be included under the NANDA-I syndrome named Risk for Frail Elderly Syndrome. This syndrome is defined as a “dynamic state of unstable equilibrium that affects the older individual experiencing deterioration in one or more domains of health (physical, functional, psychological, or social) and leads to increased susceptibility to adverse health effects, in particular disability.”[30]
Example
The components of a syndrome nursing diagnosis for Ms. J. would be:
Problem (P): Risk for Frail Elderly Syndrome
Signs and Symptoms (S): The nursing diagnoses of Activity Intolerance and Social Isolation
Additional related factor: Fear of falling
A correctly written syndrome diagnosis statement for Ms. J. would be written as follows:
Risk for Frail Elderly Syndrome related to activity intolerance, social isolation, and fear of falling

Clinical Tip: It can feel overwhelming for nursing students to determine which nursing diagnoses to use for their clients due to the complexity of nursing diagnoses. Rest assured, use of nursing diagnoses becomes easier with practice and exposure to client care plans. Refer to trustworthy sources, such as a nursing diagnosis handbook or reputable care-planning resources to become aware of current NANDA-I nursing diagnoses.
Nursing diagnoses can be viewed to establish familiarity with them on the nandadiagnoses.com website, but but be aware this is not an official NANDA nursing diagnosis site. Evidence-based care planning resources should be used when planning clientcare.
Prioritization
After identifying nursing diagnoses, the next step is prioritizing diagnoses and actions according to the specific needs of the client. Nurses prioritize their actions while providing client care multiple times every day. Prioritization is the skillful process of deciding which actions to complete first for client safety and optimal client outcomes. Through prioritization, the most significant nursing problems, as well as the most important interventions in the nursing care plan, are identified.
Client care situations fall somewhere between routine care and a medical crisis. It is essential that life-threatening concerns and crises are identified immediately and addressed quickly. Depending on the severity of a problem, the steps of the nursing process may be performed in a matter of seconds for life-threatening concerns, such as respiratory arrest or cardiac arrest. Critical situations can occur at any time when providing nursing care for clients, and the steps of the nursing process must be performed rapidly. Nursing students must have a full understanding of how to correctly analyze cues, cluster data, form appropriate hypotheses, and prioritize hypotheses to take appropriate action using clinical judgment. Nurses recognize cues signaling a change in client condition, apply evidence-based practices in a crisis, and communicate effectively with interprofessional team members.
There are several concepts used to prioritize, including Maslow’s Hierarchy of Needs, the “ABCs” (Airway, Breathing and Circulation), and acute, uncompensated conditions. See the infographic in Figure 4.7[31] on The How To of Prioritization.
Maslow’s Hierarchy of Needs is used to categorize the most urgent client needs. The bottom levels of the pyramid represent the top priority needs of physiological needs intertwined with safety. See Figure 4.8[32] for an image of Maslow’s Hierarchy of Needs. You may be asking yourself, “What about the ABCs - isn't airway the most important?” The answer to that question is "it depends on the situation and the associated safety considerations." Consider this scenario - you are driving home after a lovely picnic in the country and come across a fiery car crash. As you approach the car, you see that the passenger is not breathing. Using Maslow’s Hierarchy of Needs to prioritize your actions, you remove the passenger from the car first due to safety even though he is not breathing. After ensuring safety and calling for help, you follow the steps to perform cardiopulmonary resuscitation (CPR) to establish circulation, airway, and breathing until help arrives.
In addition to using Maslow’s Hierarchy of Needs and the ABCs of airway, breathing, and circulation, the nurse also considers if the client’s condition is an acute or chronic problem. Acute, uncompensated conditions require priority interventions over chronic conditions. Additionally, actual problems generally receive priority over potential problems, but risk problems sometimes receive priority depending on the client vulnerability and risk factors.
Example of Prioritization
Refer to Scenario C in the "Assessment" section of this chapter. Four types of nursing diagnoses were identified for Ms. J.: Excess Fluid Volume, Enhanced Readiness for Health Promotion, Risk for Falls, and Risk for Frail Elderly Syndrome. The top priority diagnosis is Excess Fluid Volume because this condition affects the physiological needs of breathing, homeostasis, and excretion. However, the Risk for Falls diagnosis comes in a close second because of safety implications and potential injury that could occur if the client fell.
